I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this proposed action would not resolve my complaint.  For your reference, details of the offer I reviewed appear below.
 The services are for my wife, Kristine.  I am the one they are billing, surely they can find our files.  I have yet to receive a phone call from this place.  I visited your office to resolve this matter with no avail.

The patient referred to himself as receiving services, without violating HIPPA, we can only state that we reviewed his records and did not find the treatment he is referring to for himself. Furthermore the date in question does not appear in our system as a valid date of treatment for this patient....

We would need from this patient, if he did not receive treatment, who did & what was the actual treatment date. We have reached out to the patient to try and clarify how we can help, however he is non-responsive in our phone call attempts.Thank you,
