the Dental Community Clinic. Wilma [redacted] was seen at our Community Dental Clinic on 9-7-16 where she had impressions made for a dental flipper and she paid a deposit for the service at that time.  The impressions were sent to the dental lab that produces our dentures.  On 9-28-16 Ms. [redacted] came in to follow up and see what the status was and leave some additional contact information, as she stated she had been reaching out to the Dental Director and was not getting a response.  I went to verify her information as was asked by the Dental Director to get updated contact information and verify it with her because the Dental Director had been calling and not getting an answer.  We followed up regularly with the lab who ultimately informed us that they would no longer be able to service us. The program director for the Dental Clinic spoke with Ms. [redacted] informing her of the situation.  Ms. [redacted] informed us that she wanted a refund.  We then began process for reimbursement which normally turnaround is up to 3 weeks.  On 10-2-16 Ms. [redacted] was informed that a new lab for production was acquired and offered her to have her services completed with them.  Initially Ms. [redacted] refused but later called back on 10-4-16 and stated that she talked it over with someone else and they suggested that she give the new lab a try.  She was given an appointment at that time to come and pick up her denture on 10-14-16 @ 9:00 AM. The impressions were sent to the new lab for process on 10-5-2016. On 10/14/16 Ms. [redacted] came in for her appointment at 9:00 AM and was fitted with her new denture as promised and she paid her remaining balance for the services.  On 10/17/16 Mrs. [redacted] came in for a follow up visit and fitting adjustments to ensure her denture was fitted properly.  She left satisfied and stated that she would be able to attend her planned events now that she had her dentures.  Again we offer our extreme apology for the delay, inconvenient and frustration the incident caused by our vendor’s service cancellation. Sincerely, [redacted]COMPLIANCE CONSULTANTStudentOmbudsmanandLead Title IX CoordinatorEDUCATION CORPORATION of AMERICA
