Have been trying since July to get a rebate on Allergan Opthalmic eye drops. Mailed in all my papers on July 25, 2018. Waited over 6 weeks and didn't receive a rebate. I finally called on October 1, 2018. They said they never received by paperwork. They requested I scan my parer work in which I did. . Called on October 10, 2018 to double check that they claim was indeed processed and they said yes it was processed on October 2, 2018. Called on November 19, 2018 and they said yes I should have received a check but there was no address but they will now issue the check. Called on November 30, 2018 and they said they would have to send another form for the check to be processed and it should be sent out in a week. Called on December 13, 2018 and they said the check had an error in my address. So they reissued a check and it was set to be mailed on December 17, 2018. Called on December 27, 2018 and they said the supervisor would call me back. The supervisor did call and she said the check would be sent out the first week of January 2019. She said she would call me back and let me know if it did go out. She never called back and now when I call her personal number it goes straight to voice mail. Left a message on voice mail and never received a call back. Called again today, January 16, 2019 and spoke to Georgette and she said she would get to the bottom of it and would call me back tomorrow morning to let me know when the check was released. Ugh!

I've been receiving Botox injections for chronic cluster migraines for several years and their savings program is a joke. It's been a nightmare from its inception. Always have to call and upload documents over and over just for claims to be rejected. I'm emailing them directly and going to threaten to contact the Attorney General as well as the BBB and see where that gets me.

I've had to request a check 3 times for my first treatment this year and now I'm on my 4th request for my second treatment. Where do the checks go? Why is this such a hassle? I rely on this for my payment to the doctor for my chronic migraines. And why is it called a savings card program, it is now a check mailed to you not a card that is reloaded. Please fix the time issue and "missing checks".

I have applied for Allergan's botox reiumbursment card for chronic migraine and been unable to receive any reason why I haven't received it in the mail after two months. I have made numerous phone calls and have been told that the paper work is in order. The website states that the card will be received within 7 to 10 business days.

It took me 6 months to receive my card (this was almost 2 years ago). After numerous phone calls and being told every time that the card was being mailed, I finally had to threaten to contact the Attorney General for fraud. Amazingly the card was in my mailbox a week later. Just today, I received notice that the program is changing and cards will no longer be used. Payments will be fulfilled via mailed paper checks. This makes absolutely no sense. This makes it twice as difficult, on both sides. I wonder how much longer the program will be around now.

Review: I had implants in January 10, 2008. I bought warranty with Allergen that covers the implants and $2400 reimbursement for medical cost. Serial numbers:

Left: [redacted] right: [redacted]. Style# 68. On October 1, 2014, one of the implants leaked and I had to have surgery to remove them. I consulted with the doctor in San Diego that did the implants but it was too expensive for an unexpected cost. I went to Tijuana and had surgery with Doc [redacted] to explant them. This was consulted with allergen and approved. Doctor had implants sterilized and then I immediatelly fedex the implants for replacement and also reimbursement. To this date, December 5th, two months later, I'm still waiting for reimbursement and seems like every time I call Allergen nobody is concern in giving some sort of priority as this has to do with more surgery. Incisions to remove implants will be totally healed by the time Allergen decides to act.Desired Settlement: I am requesting expedited service due to the nature of the issue.

Review: I have tried to resolve this numerous times, but my concern has not been resolved. I have purchased their product "Restasis" for many years. You have not follow through on their independent rebate redemption program originally signed up for. My ID number is [redacted] and my group number is ed [redacted]. I am requesting that they follow through by sending me the appropriate rebate. The only rebate card that they have ever sent in 2012 did not work. I followed up with them and requested that they send me a new one, which they never did. I have been sending them the appropriate forms after purchasing restates since that time and still have not received any rebate.

Thank you for your help in this matter.Desired Settlement: I am requesting a refund/rebate for all the units of Restasis that I have purchased since I originally signed up for their rebate redemption program.

Member now has a card and 'directions' on how to use the card going forward. This should hopefully resolve any further issues for this customer.

Review: I receive botox injections due to chronic migraines. I qualify for Allergan's "Botox Savings Card" which allows me to turn in receipts for my botox in exchange for a Visa card where I am reimbursed for portions of my out of pocket costs for these medical injections. I have done this for several years. However, this past year a new company began handling the savings card and I have repeatedly had issues getting my reimbursements. I have returned my required documentation (claim for, EOB, receipts) in the required time (3 months) via mail, with the envelope provided to me BY THE SAVINGS CARD COMPANY. Each time, they claim they do not receive my info. They then provide me with a fax number & ask me to fax the information. I do so. I then get a letter in the mail stating my claim is rejected for not turning in the required forms, which I have done. Twice. So I send it all AGAIN & then am told my claim is rejected bc it's now past the time frame. This has now the 3rd time I've gone through this & each time I've confirmed I sent the correct information the first time. The first 2 times, they finally paid me a portion of what I was actually owed. I still havent heard back from them again for my third claim. If you call, you can't get a live person & have to leave a message and wait (hope) someone calls you back, days later. Why offer someone a reimbursement plan if you have no plans of actually reimbursing anyone?Desired Settlement: I expect to be reimbursed for my last claim. There is a yearly maximum that I have no met. I have turned in my correct, required documents to them three times now since August 2015.
