I am responding to your August 11, 2015 letter regarding [redacted]'s complaint that we are not covering [redacted] and [redacted] on his plan.
I have reviewed the coverage for [redacted] and [redacted], and can confirm...

that both [redacted] and [redacted] have coverage that has been in effect since December 20, 2004. Additionally, there have been no claims denied for either customer stating they did not have coverage with CHLIC.
We did request verification of other coverage information for [redacted] and [redacted] as Cisco System, Inc. requires this information once a year. Our phone records reflect that we updated [redacted]'s coverage information on March 30, 2015. On May 1, 2015, we updated [redacted]'s coverage information to reflect no other coverage. We have paid all claims received for both customers.
