Your daughter was originally seen for a New Patient Routine Comprehensive Eye Exam on 11/11117. Our usual and customary fee for a New Patient Routine Comprehensive Eye Exam is $145.00 and your daughter's vision insurance plan, Eyemed, has no patient copay for this service. The Claim Summary we...

received from Eyemed indicates $145.00 as the total charges billed for the Date of Service 1111 l/ 17. When an insurance claim is filed, we are required to indicate the Usual & Customary Fee for the type of exam. Eyerned paid IL?? $50 only, and $95.00 is vvritten off. There was no charge to you for this exam. This is the only claim we have sent to Eyerned for your daughter. We would not send a claim to Eyemed for any conUJ.Ct lens services or disposable contact lenses for her because there is no coverage for those on her specific plan. During the routine exam, you indicated there was interest in trying disposable contact lenses for your daughter, although that was not pursued on 11/11/2017. A New Patient Routine Comprehensive Eye Exam fee does not include any contact lens services. The appointment you made for your daughter on 12/22/2017, was for a Torie Contact Lens Evaluation/Fitting, and an Insertion & Removal Class. The class is required for all new wearers of contact lenses. There are separate fees for 1hese additional services. At your request, a staff member supplied your spouse v..ith our fees in writing for these specific services on 12/27/17. Our Usual & Customary Fee for a Torie Contact Lens Evaluation/Fitting with a New Wearer Insertion & Removal Class is $100.00. This should have been the charge for the services on 12/22/17, but you were only charged $80.00 which was paid on 12/27 /17. Your daughter's particular Eyemed vision insurance plan does not offer any coverage for contact lens services or for the disposable contacts you ordered for her, and that is why you would owe for them. We discovered that the Torie Contact Lens Evaluation/Fitting Fee was applied to 11/11/17 date of service instead of the 12122/17 invoice in error, therefore it was not originally on your 12/22/17 invoice. This has been corrected and we believe that was what caused the confusion for our staff member when you spoke with her. We understand that insurances are complicated. We sincerely apologize for not making it clearer to you and for the invoice error causing the confusing explanation you originally received.
