Dr. E.J. Bartolazo

Review: Received a billing statement from Dr. Bartolazo's office with no information except that the amount was past due. The date of service was not listed. The services performed were not detailed. No information regarding insurance submission was noted. The "bill" is a photocopy of several pieces of paper. It is unsigned and not on any letterhead.This is my first notice that there is an outstanding amount. I have not received any prior bill.I moved from NYC about two years ago and cannot understand why this notice would be arriving now when I have not seen the doctor since I moved.Furthermore, since the "bill" looks cobbled together and is wholly unprofessional.Unfortunately, this is the second time I have received such a notice from the doctor. With my move, I am unable to find the previous one. When I received it, I called the doctor's office to get more information, like the date of service or the insurance to which it was submitted. As I recall, it took some time to get that information and that the bill was for services performed two years prior.My complaint is not about the amount due but rather about the time it took to generate a bill and the lack of verifiable information on it.Desired Settlement: I would like a replacement bill, properly generated and containing the appropriate information. It should have date of service, services performed and information about how it was handled by my insurance.
