For both of these patient's cases (Case *** and Case ***) the insurance information that was provided to us when this patient registered was not sufficient for us to submit a claim to her secondary insurance carrier, ***. This insurance information was requested on multiple occasions
but was never provided.Upon receipt of our statement dated 02/03/the patient called to request that we bill her *** insurance and she was told that we did not have sufficient information to submit the claims. She stated she would have to drive to Jefferson Barracks to get the information.On 4/25/a final demand for payment letter was sent to the patient and on 6/8/a follow up phone call was made, a message was left on her voice mail to contact us with the required insurance information. The patient called us on 6/30/saying that she did provide the information to us via certified mail but she could not tell us where it was sent or who signed for it. She said she was going to send the information via certified mail again. It was never received.Another telephone call was made to the patient on 7/11/stating the required information was not provided and that payment was needed to prevent any further collection activity. In total the patient received eight billing statements ranging from December through July 2016. On 7/15/approval was given to place the account balances of $and $into collections with ***
