First - we made a SIMPLE request for this company to provide the documentation (they surely must have) showing that we DEPOSITED every check issued because that will 'prove' as it were that if it was claimed to have been sent - then IF we received it - we would have deposited it. Our records show one reimbursement has never been made - they say all reimbursements have been sent.
Secondly - Note in their response the number of times we traveled to the testing facility. WE WERE CALLED BACK for re-testing ONLY _ONE_ TIME according to OUR records - therefore that (by THEIR count) has us making one more trip than they have reimbursed. All other trips were for testing according to the schedule THEY set forth.
This matter can easily be settled.
All the company has to do is to provide the requested documentation SHOWING EVERY CHECK THEY SENT HAVING BEEN DEPOSITED INTO OUR BANK - and again note - they say we made TWO trips for which we we were NOT reimbursed - BUT OF THOSE ONLY ONE (the trip for re-testing) was made that would not have been eligible for reimbursement according to their rules.
That STILL LEAVES ONE VISIT which according to our records show was never reimbursed - and it SHOULD have been and still should be given that it was to comply with a scheduled test they scheduled.
Finally - please note that all CAPS are for emphasis, not "shouting".

Dear Revdex.com Customer Relations Advocate,
Thank you for the opportunity to respond to Mr. [redacted] concerns. I think I had outlined the difficulty in my previous email.
 
I think the misunderstanding remains the number of payable “times through the door” which, as noted, are strictly regulated by the trial sponsor.
 
There were a total of eight times “throught the door”.
Two of them were not payable according to the study protocol.
1.Visit 3 was in in two parts (he came through the door twice and gets paid for just one).
2. He was called back on one occasion for repeat of abnormal labs. This “time through the door” is not payable.
 
That is how we come to six payable visits despite eight times through the door.
I know that Mr[redacted] wants our documentation that he deposited every check. Neither our accountant nor partners feel comfortable sending copies of our bank records. I believe Mr[redacted] does say he got six payments so, we do not disagree on this.
 
Once again, I appreciate your help in this matter and the opportunity to explain our misunderstanding.
The Customer Relations Advocate I spoke to today was very helpful and I appreciate her input.

I am in receipt of the complaint filed by Ms. [redacted] on March 13, 2015 regarding a form she states wasnot filled out by our provider, Our office policy is that the patient must bring the form to theirappointment to have the form filled out. Ms. [redacted] was told that she needed to schedule...

anappointment to have this completed. She chose not to make an appointment for 2 month and only afterthat appointment d¡d she request a refund. The refund was sent to the patient and she has since cashedthe check. Please, let me know if I can be of any further assistance.Sincerely,
