September of 2013, Delta Dental of Arizona received a claim for orthodontic services.  The claim indicated that the patient had insurance coverage from another carrier.  Based on the information provided by the dentist, Delta Dental of Arizona was unable to determine which insurance was primary and could not properly process the claim.  Delta Dental of Arizona then sent a request for this information to Ms. M[redacted] and notified the provider that the claim could not be processed without additional information.  The information requested was not submitted.  In fact, the information needed to process the claim was not submitted until September of 2015.   Under the rules established by the employer group’s insurance plan, no claims older than 1 year can be processed and paid. Nevertheless, Delta Dental will pay the amount requested.  We value our customers and want their experiences with Delta Dental to be excellent.  We regret the difficulties Ms. M[redacted] encountered. Please let me know if you need additional information. Anne B[redacted]
