HOW DOES THIS *** BUSINESS GET A .US WEB ADDRESS? We thought we were dealing with the social security agency. Yes, there are disclaimers in corners and fine print at the bottom of the website, but the main windows state that their service enables you to apply for a replacement card online. IT IS NOT POSSIBLE TO APPLY FOR A REPLACEMENT CARD online. What the business actually does is charge almost $40 for a label. What they don't tell you is that, IF YOU FOLLOW THEIR INSTRUCTIONS, YOU ARE PROBABLY BREAKING THE LAW. The package says to send your photo ID--that is usually your driver's license--in an envelope addressed to the social security department. Either you don't need to go anywhere for a few weeks, or you will be driving without a license. Finally, the website does not warn you that there is an arbitrary three-day cancellation provision.

E-forms' misleading website appears to be the official IRS site. I paid $148 for acquiring an EIN that could have been obtained free from the IRS.
On January 12, 2020 I went to the E-forms website, which appears to be the official IRS website, to obtain an EIN for my recently deceased mother's estate. I paid $148 to E-forms when I could have easily obtained the EIN from the actual IRS website at no charge. This is a fraudulent business practice because the misleading E-forms website masquerades as the IRS website. I just realized today that I was conned into paying unnecessarily to get the EIN (which I haven't received yet), and I could have already had the EIN because the IRS provides it immediately on their website. I fortunately realized I was misled before I actually received the EIN and called the E-forms customer service number to speak to a representative. I was able to cancel my order, and E-forms is supposed to be refunding my money.

Desired Outcome

E-forms needs to modify their website to make it more obvious that they are charging for a service that consumers can do themselves at no charge.

We are a legitimate business helping consumers obtain their business EINs. When a consumer runs into a problem with their EIN they don't want to spend 2 hours on the phone with the IRS since most are business owners and do not have the time, thus they use services like ours to get things completed.

The office of Social Security denied the paperwork that was sent in provided from this company.
I filled this paperwork out under the false assumption that this company was associated with SS (this was my fault) to have my name changed on my social security card since I was recently married. After 2-3 weeks I received a denial letter from the office of SS so I went into the office with the letter and utilized the form that was originally sent in. There was nothing wrong with the form. So I paid for the services from Easy Apply and still had to go into the office after their program did not fulfill its job. I sent an email stating that I wanted to refunded to the company because their services failed and they said they only had a 3 day policy which does not make sense given the amount of time it takes to mail in a form is longer than 3 days.

Desired Outcome

I requested and was denied a refund because it was 3 days after the purchase. However there is no way to know whether or not the information is going to be accepted until it reaches SS. There was no issues with the paperwork information so there should have been no reason that the name change request was denied.

Our website states in 3 sections of the home page: top, midsection, and footer, the fact that we are not affiliated with the Social Security Administration.
In no way do we try to impersonate any government agency. We do not hide our phone number, our location, or the fact that we are not affiliated with the social security administration.
The top section states: "Not Affiliated with the SSA"
The mid-section states: "While the Social Security Administration allows you to request a social security card at no cost, e-forms is a paid service with the objective of simplifying the process, eliminating guesswork, and providing detailed instructions about how to fill out and where to mail government forms. Easy-Apply is not affiliated with nor endorsed by the SSA" (this in bold letters).
Our footer states: "It is possible to obtain an SS-5 form at no cost at ssa.gov. "
Also, the checkbox for the customer to acknowledge the terms and conditions before placing the order also states: "I also understand that the Social Security Administration supplies blank forms and instructions for new and replacement cards free of charge. Nevertheless, I wish to use Easy-Apply services with personalized instructions to fill out the SS-5 form alongside all additional services provided herein."
Many people find it difficult to find instructions and to fill out government forms, thus seek the help of assistants like ours. Unfortunately, the information provided only states that the form was mail in. Therefore you were denied as the instructions were not followed; only original documents are accepted with all forms. We are not liable if a consumer makes a mistake or if the SSA chooses not to process the application because they have identity concerns about the individual.
As a one-time courtesy we are issuing a refund. Please allow 3-10 business days for the amount to reflect in your account.

They are refunding me (though they still claim that I did not send in all required documentation and the office of SS says that I did).

Thought this was the IRS site and filled out online application for an EIN.
I thought I was on the IRS site. They use the IRS logo on the page. I was originally on the IRS site and have no idea how I was directed to this site. I filled out an application for an EIN for my business and they now have all of my personal information such as email, SS # (which is required for the EIN) address etc. once the application was submitted the next page wanted payment. I did not fill any of that out but now they have all of my personal information and I'm afraid they will use it to steal my identity. I am going to have to file with the social security for a new card and now change my email etc.

Desired Outcome

I have no idea what the resolution would be. If my identity is stolen I will know exactly where it originated from. This business.

We do not hide out contact information or location which can be located at the bottom of the website as well as the contact us link.

They did not address my concerns about the fact they have all the personal information such as my social, dob, email and address all of which can be used to steal my identity. They fraudulently make their website look much like the IR as and use a logo on the top of the EIN application that looks very similar to the IRS. I had to put freezes on my credit because they have all of my info and it does not make me feel good knowing they do when they are so fraudulent and scammers.

The form I filled out went through even though I did not proceed with payment. Once that popped up I knew they were fraudulent. So where did all of my personal info go? It doesn't just disappear into thin air!

This has nothing to do with cookies. I know how cookies work. I filled out a form with all of my personal information and hit next. Then the payment page came up and I did not fill that in. Where did the completed form go with all of my personal information.. I'm not talking cookies a firm with personal info gets sent somewhere when you hit next or submit. Is it sitting on your server? Does someone at your business have all of my personal information? Again I'm talking about a completed form not cookies

A form is not completed if a payment was not made.

Just like when you go to a site to purchase something, you will not get the item delivered to you unless you make a payment. If you call customer service they will have no clue who you are since the process was not completed.

I'm not talking about not getting anything. I'm talking about a firm was submitted with personal al info they have now and can use it to arm real my identity. What happened to all my personal info that was submitted? You are avoiding answering the question. You are a fraudulent company because you can't answer.

I went to this site thinking it was the social security web site to apply for a replacement card for my husband and it is not.
I went to this site to apply for a replacement social security card for my husband and did not relize until after I was charged $39 that is is not the social security website. I just paid them to do something that I could have done for free. And they are just a middle man and that is not what I wanted when I tried to get the forms.

Desired Outcome

I want my $39 refunded because I did not use this service. I went to the actual government website and filed for myself.

DO NOT USE THIS COMPANY. THEIR WEBSITE IS "EASY-APPLY.US". My personal experience with this site is that they charged me almost $40 for a service that is actually free and better done in person because you have to show official identification. I had to get my child's social security card replaced and they showed up at the top of my Google search when I was trying to find out how to get a replacement card. They set up their website with the same info as the Social Security office, which makes their site seem like they are affiliated with the office, AND THEY ARE NOT. Actually my form (that this company sent me) was rejected by the SS Office and that's when I found out that 1) this company is not associated at all with the SS office 2) getting a replacement card is actually FREE 3) and should be done in person because original identification will have to be shown of parents of minors. None of this info was clear on the forms I was sent. DO NOT USE THEIR SERVICE.

Please consult our Privacy Policy to understand how we process" which is also a disclaimer that the customer has to check off before proceeding that they understand and have read the statement. Original documents are required by the SSA which is statement that comes directly from their office, "Important: All documents submitted must be either originals or copies certified by the issuing agency. We cannot accept photocopies or notarized copies of documents. Get a certified copy of a document showing a birth, marriage or divorce that took place in the United States.
Note: If you are filing an application on behalf of someone else, you also must show us evidence of your relationship to, or responsibility for, that person. You must also show us proof of your identity." The documents can either be mailed or taken into an SSA office. We assist with the mail in process and are convenient for those that are unable to make it into the office. We apologize that the SSA did not accept your form, which is the same form they use online if you were to print out the form. Lastly, our website does not look anything like the SSA site.

So, I am confused. If you are requesting and providing the same information that the Social Security office is requesting, plus NOT providing any additional service EXCEPT providing a USPS label what exactly are customers paying for. For you to pre-populate the information on this simple form that can easily be retrieved from the Social Security Administration website here: https://www.ssa.gov/forms/ss-5.pdf

These are your claims on your website and below is the reality.....

They don't tell you up-front that there is a $39 fee for their "service." What you get for that money is a form which you then have to print at home and mail to Social Security, along with your driver's license or passport. They do provide a mailing label for your money.

I cancelled my order and will go to my Social Security Office for a replacement card.

This company should be criminally investigated. They claim to help you get your replacement social security card in 5 days. They charge you $39.90 and within 24 hours the credit card you gave them is BREACHED!!!!! Now they have your social security number which they claim they don't keep. When their merchant ID comes on your credit card statement it has a number no longer in existence... and their company is listed as easy-appli ( notice not easy apply). I have contacted the Federal Bureau of Investigation along with all the other agencies that need to know about their fraudulent behavior. I won't be satisfied until I see them listed on the front page of the Wall Street Journal. The entire company needs to be prosecuted!

We got two pest calls from this outfit today, one under the caller ID E-Forms.US and the other one no caller ID. Both were from a phone number which is not the number shown to the right for their business. Since we did not contact them for their services, it appears they are trying to drum up business with telemarketing calls. Go Away!

E-Forms.us is not a telemarketing company. We do not make outbound sales calls to anyone. The only time our company contacts a customer is because we have a missed call from that number or there is a problem with an order that was placed. You are not in our system as a customer so we would have no reason to make a call to you unless you somehow found our number and called us so we are returning the call.

If needed, we have proof of EIN delivery as well as the recorded call where we can hear the voicemail of customer playing and representative leaving a voicemail on 6/7/2019.
No refunds are given after the EIN is obtained on behalf of a customer since services were rendered. As a courtesy we can issue a partial refund of $74.00 since services were provided.

Misleading website that charges for a free service
I went to the SSA website to file for a replacement card and was directed to this website for some reason. The website looks just like the government site and is very misleading. Once I filled out all the information and paid the $39.90 fee, it said the website was not affiliated with the SSA. I tried to send an email to customer service twice, but it said the server did not exist. I tried to call, but it said it was closed. I then filled out a cancellation form online, but have yet to hear back. The website is very misleading and looks just like the SSA website. They are charging for a service that can be done for free and misleading consumers.

Desired Outcome

I would like my money refunded in full. I will also be monitoring my SS number and credit since they have my information.

I usually am very careful but was rushing to replace a card and got taken thinking on was on the SSA site. This profit site should be shut down and google notified that their click line should not say 'social security administration'. Business claimed could help me quickly get my social security card replaced. I paid fee, never got card, and social security office said to report.
This website looks very official. It claimed it would provide an expedited service to get me my social security card. I wanted to get this before I renewed my driver's license. I called and was assured this was legitimate, but I paid the fee and never received a card. Social Security said they prey on senior citizens, this is not an expedited service provider, and had me cancel my credit card, report the agency, stop payment, and then they gave me a new social security card.

Website misleads individuals applying for an EIN from the IRS, making itself appear like a government website and charging for a free service.
On Monday, May 20, I did an Internet search to find out how to apply for an Employee Identification Number (EIN) from the IRS for my new small business. One of the first websites that popped up was https://ein.e-tax-filings.com/. This website is misleading enough so that unsuspecting consumers will think they are applying with the government. The website has been designed in the same colors and font as the government's website. I was charged $183 to apply for a next-day EIN number. Today, I found out that this service is free and just as easy to obtain from IRS. One only needs to apply online and can get the EIN immediately, with no charges. I feel that this business is taking advantage of first-time applicants like me who miss the small details. I called them to request a refund and they only gave me $83 back because they said my service had been completed.

Desired Outcome

I am seeking to have the rest of my money refunded--$100 (they are in the process of refunding $83). And I also think this website needs to be shut down. I understand that legally they may meet the requirements, but it is still clearly created to mislead people.

This company's website misleads people into thinking they can apply to change the name on their social security card online and they charge a fee.
I used this site on 4/29/19. It claimed that I could apply to change the name on my Social Security card online and then mail the application and necessary documents to the nearest Social Security office. They charged a $39.00 fee for this, which was charged to a credit card. I decided to take the documents to the Social Security office instead of mailing them. There, I was told that you can't apply online, and that it is free. Now I am afraid because that company has my information.

I want a refund of the $39.00. I want some way of knowing that my information (SS#, address, credit card# etc) has been expunged from their records. I think this company is committing fraud and should have to post a large, easily noticeable disclaimer on their website saying "This is NOT the official Social Security website. Somehow I got redirected from the ssa.gov site. I called when I saw that I had to pay.The Info given to me was wrong. SCAM
On aPRIL 22, 2019 my daughter in law went on line to apply for a replacement card for me from Social Security. I am 94 and can't travel. She got redirected from SSA.gov to their site. When we saw that I had to pay we called and was told that it was easier then going to the SSA office and that I had to make an appointment at SSA and that it could take weeks. We went ahead and paid and was told I didn't need any other document to go with my SSA application for a replacement card expect a letter from my doctor stating my date of birth, address and his signature. We sent my other info to SSA and got a letter back that I needed to go in person to get my card. I got nothing for $39.90 except lies.

Was told by SSA that I was scammed. I was told to close my credit card account to avoid any more fraud. This company responded to me that I will be reimbursed but they have my now old card number. I asked fir a check instead. I have not heard back.

Thank you for contacting E-Forms. We apologize that the Social Security administration scared you into closing your account. We only charge for the service that is selected. Your SS information is not stored into our system and we are a real company with a real location. Please visit our site, we do not hide any of our contact information.
The resolution desired per the customer was to refund the credit card. We issued a full refund to the card ending in 3567 American Express on 05/02/2019. We will not be issuing a check as a refund was already given.

I expected to receive a replacement social security card for my foster son. I paid $39.90 and was only sent paperwork that social security gives free
on 4/23/2019, I paid $39.90 from my Visa card to receive a replacement social security card through e-forms.us. On that same date, I read on the order confirmation section: "Social Security Card Replacement, Instant E-Package $39.90, Total $39.90.The order # was #XXXXXX." The directions were to send in an original drivers licence or passport, which is ridiculous. I believe that this is a scam company and I demand a full refund of $39.90.

E-forms is a private company that provides paid assistance in preparing and filing documents. We do not charge for the form itself, but for the assistance in its preparation. Our charge is a flat fee of $39.90
Our website states in 3 sections of the home page: top, midsection, and footer, the fact that we are not affiliated with the Social Security Administration.
In no way do we try to impersonate any government agency. We do not hide our phone number, our location, or the fact that we are not affiliated with the social security administration.
The top section states: "Not Affiliated with the SSA"
The mid-section states: "While the Social Security Administration allows you to request a social security card at no cost, e-forms is a paid service with the objective of simplifying the process, eliminating guesswork, and providing detailed instructions about how to fill out and where to mail government forms. E-forms is not affiliated with nor endorsed by the SSA" (this in bold letters).
Our footer states: "It is possible to obtain an SS-5 form at no cost at ssa.gov. "
Also, the checkbox for the customer to acknowledge the terms and conditions before placing the order also states: "I also understand that the Social Security Administration supplies blank forms and instructions for new and replacement cards free of charge. Nevertheless, I wish to use e-forms' services with personalized instructions to fill out the SS-5 form alongside all additional services provided herein."

We have reviewed the order XXXXX and have found no calls or e-mails from the customer requesting a refund. This is an unfair practice for a consumer who has all of the merchants information (as the customer stated and e-mail/e-package was received) and not even attempt to contact the merchant to find out what services were offered. We have issued a full refund, please allow 3-10 business days for the amount to reflect into the same form of payment that was used.
