This memo is in response to Mr [redacted] complaint with the Revdex.com reference number [redacted] Mr [redacted] stated that we didn't give him back the copies of his ID and Social Security card; we do not make copies of I D's and Social Security cardsWe ask for them to verify the identity of the prospective donor fo llowing the Food and Drug Administration and Plasma Protein Therapeutic Association guidelinesThe information that is collected from the ID and Social Security card is used to create the prospective donor a donor chartThis information is used for in-house purposes only and is not sold or distributed to third party vendorsIf the potential donor does not qualify for the donation program, then the donor chart is archived in storage with limited access.Mr [redacted] mentioned he would like his blood sample that he providedUnfortunately, this is not possible to give back a small sample of bloodThe sample that was collected was used to type Mr[redacted] 's blood to verify if he qualifies for the donation programAfter the sample has been tested and typed, it is then discarded into bio hazard waste and properly destroyed by a licensed medical waste company.All employees working at Ked Plasma-Somerset labs have gone through extensive confidentiality training during orientation and then receive annual training after thatIn the plus years of operation, this location has never given out or sold donor information that would put some one's identity in jeopardy.Mr [redacted] can be assured that his information is treated with the same care and importance as all our donors.If you have any questions or need any more information, please contact me at [redacted] .Thank you Chad Barker Center Director Ked Plasma-Somerset Labs Phone: [redacted]
