It took over 8 months after 16 requests to receive a copy of my medical file. She was very uncooperative in releasing the documentation as by law patients have a right to receive their medical file. She also states that I have Parkinson's Disease in the file, which I don't have a diagnosis of nor do any of my other medical files submitted to her state that I have. Not to mention, Parkinson's Disease was never even discussed with her. She also states in the file that I chew gum, which is impossible when I have no lower back right teeth or upper back left teeth, making it impossible to chew gum.
