Treatment Summary:The patient was seen by me multiple times between 09/18/2015-11/24/diagnosed her with Severe Periodontal Disease, which is an inflammatory condition within the supporting tissue of the teeth, progressive attachment, and bone lossThe first step in treating her involved a
"deep cleaning" (= Scaling and root planning)This is a thorough cleaning of her teeth, above and below the gum levelHer "pockets" were also irrigated with a special disinfect solution and a local antibiotic was placed in some of her deep pocketsTo control her teeth grinding and sore masticatory muscles, impressions were taken and a custom-made night guard was fitted.I saw her multiple times at the office, often on very short noticeI was always very attentive and always completed thorough examsShe kept Complaining about pain from different sides of her mouth and attributed that to her night guardShe stated that she read a lot online about how bad nightguards can be and decided her night guard was harmful.When examining her, made sure that there were no problems related to her nightguard useexplained that the pain may be related to her untreated advanced periodontal disease or to another dental problemThe patient expressed her philosophy of preferring Conservative treatment only, with minimal interventional treatmentI reimbursed her for the night guard, and the patient was supposed to follwith me in the near futureShe stopped by the office after several weeks to inform me that she had a root canal on one of her teeth and is no longer in painAs far as I recall, I never saw or heard from her afterwards.Billing Summary:The insurance, ***, took a year to pay the services from 09/18/Insurance requested additional information, which we provided, and the claim was finally settled on 09/06/2016, with a patient final balance of $337.88.In reviewing the patient's transactions, we realized that the insurance was not reimbursed for the return of the night guardWe called her insurance on 11/14/to address this issue and *** insurance confirmed it and agreed to apply that payment from the night guard towards the deep cleaning charge (that was originally not covered by them as she maxed out), therefore that will reduce the patient's remaining balance to: $194,08.Considering the inconvenience and Pt's frustration, DrSperling decided to settle this case out of good will and she is willing to waive off the patient's remaining balanceWe are sincerely sorry that the patient had a negative experience with our officeWe hope that waiving her balance, effective immediately, will assuage her aggravation.Below is a brief summary of her payment history:O The patient initially paid $(two equal payments of $on 9/18/& 9/22/2015) for treatment, including: the night guard, deep cleanings, irrigation, and local antibiotics.OWe reimbursed the patient Son 12/15/because she no longer wanted the night guard.O To date, the patient has paid our office a total of $(=$990.40-697.20).O The total fee for treatment (excluding night guard) was $833.00.Insurance paid: $345.72.Patient's responsibility: $She paid $293.20, therefore her remaining balance is:$
