Complaint: ***I am rejecting this response because: It is completely falseFirst of all, the Dr that I saw at *** *** ***l opened my L and I claim for me dated July 12, That is in the system for L and IMy claim had not been approved yet but it was in the system2nd, I informed my provider as soon as I saw her that I had applied for L and I and she told me they would more than likely not accept me3rd, I informed *** I had been approved for my L and I on July 20th when I went to my apptTo receive my results and he still saw me in the office knowing I had been approved4the I have NEVER been late on a bill with Quality CareI have paid my bills off as soon as I received them in the mail as soon as my insurance paid their partAnd finally, he cussed at me, it was a threatening position that I was placed in and I have it recorded I feel threatened by him in his authority as a caregiverI have no relationship with him as he is claimingHe was aware I had applied, his staff was aware I had applied and no one once told me that they did not accept itHe was fully aware when my claim had been accepted and he still saw me n the clinicHe even tried billing L and I, why would he bill them if it he did not accept responsibility? Also my chart notes are wrong for my initial exam with my drIt says I am training for a triathlon which I do not even know what that isL AND I gave me copies of those chart notes because *** submitted them to L and I at their request. I wanted to attach the profile from L and I for when I and the hospital filed the L and I for meI also wanted to attach proof of the referral given to me by Quality Care, they did not refer me to *** *** *** to file my L and I on July They referred me to a Neurosurgeon to perform surgery on meCan I attach or fax to you?

I am in receipt of two letters from you,one dated June 8th and one June 9thWhile J do not agree with *** ***, I am going to write off the balance on her account
The L&I Report of Accident she submitted to you was time stamped 07/18/at 16:52, when she filled it out online with Department of Labor and IndustriesShe claims she filled this out on July 12, in her previous complaint letter I think this pretty much tells the whole story

I am going to respond to this complaint letter by addressing each issue. I will underline each issue and label it with a number. In this letter I will discuss each number.
1. [redacted] did not sign up for L&I on the 12th of July. We referred her out to [redacted] to open her L&I...

claim. We referred her to them on the day she got her MRI results, July 20,2016. If she had even presented an L&I number at our office (which the clinic that signs you up will tell you to present this to anyone who gives you care related to the L&I claim) we would not have seen her. She is the first person in over 10 years that has done this.
2. Our website has not been updated since 2012. I cannot find where she saw we accept L&I claims. Several years ago L&I decided that to be one of their providers, a person needed to attend one of their classes. None of our providers will take that class. Yakima only has four designated clinics that can open L&I claims due to L&l's rule. We have not done L&I work since 2002. I do not think she saw we accept L&I on our website.
3. [redacted] probably did tell her this.
4. Hard to be approved when you haven't filled out the paperwork yet. Remember we referred her on the visit of July 20, 2016 to [redacted] to open her L&I. When we referred her out for her MRI we had to get the approval from [redacted], her health insurance.
5. I agreed to see her because she is the sister in law to a friend of mine and I didn't want to be a jerk and not see her. At this time she never presented an L&I number, of course because she didn't have one yet!
6. It is true; L&I sent me several threatening letters and a few phone calls. The letters threatened to take away our L&I provider number if we kept billing [redacted]. We do not have an L&I number so they can do whatever they want. We do not participate in their program.
7. It is true I was mad at her and [redacted]. [redacted] is notorious for taking money back when they should not. [redacted] has the most complaints filed with the Insurance Commissioner's Office as any insurance in Washington. I probably did cuss when I talked with her on the phone. This is not the first time I have had to deal with [redacted] not wanting to pay a medical bill. Last time it was with a pathology lab that she did not want to pay.
8. I will not send [redacted] anymore statements. Her account will go to collection. She is the one who is ultimately responsible for her healthcare and choices. If [redacted] would have presented the L&I card to our office we would not have seen her. She did not have this card because she did not open an L&I claim on July12, 2015 as she said she did. Had her L&I claim been "accepted" by L&I as she states in #4, her surgery would have happened right away. [redacted] did have surgery, but it was delayed by 6 weeks due to L&I investigating her claim.ln closing,we did provide services to [redacted]. We gave her medicines, referred her for an MRI, referred her to a neurosurgeon, and referred her to an L&I doctor. We need to be paid for services rendered.
