In regards to a consumer’s complaint from [redacted], in regards to Quirt Family Dentistry –Schofield WI. At Quirt Family Dentistry, we strive to provide each of our patients with the highest quality health care experience possible and take pride in committing to each patient’s oral health.
Upon receiving this information and reviewing this account, Mr. [redacted] was listed as the guarantor and Head of Household on this family account due to the patient (his son) being a minor when making Quirt Family Dentistry their dental home. In addition, Mr. [redacted] also was the carrier for the insurance to properly bill for his son’s services, causing us to list him as the head of household May 12, 2011.
We appreciate this being brought to our attention, as we have made changes to the account to reflect his son as the new guarantor of his account. The account now reflects the last known address listed on his son’s health history form.
Please be informed that our records indicate that Mr. [redacted] is listed on his son’s HIPAA acknowledgement of privacy form, thus not making this a violation of HIPAA.

I have reviewed the response made by the business in reference to complaint ID [redacted], and find that this resolution is satisfactory to me. However I don't agree with the fact that the address for the bill continually changed to my address even though I never informed them of my address change.(the address changed twice) Also, I already called them regarding this complaint so the fact that they are now doing what already asked is ridiculous. I would like it posted that they treat their customers disrespectfully, and I should never have had to contact the Revdex.com for such a simple response.
