the work necessary to repair the tooth were quoted to[redacted]. Estimates are based on the information the insurance companyprovides our office. A financial agreement for the services completed thatday was reviewed and signed [redacted].The insurance claim was submitted at completion of the restoration. Theinsurance company paid their portion September 15, 2014. [redacted] wassent a statement the same day. On October 15, 2014 [redacted] wascontacted by phone and email concerning the balance on the account, noresponse was received. On November 7, 2014 a statement was sent to [redacted] with a notice to contact us by November 17, 2014 to avoidcollections; no response received. On November 19, 2014 [redacted]account was sent to collections with the collection fee added at that time.On December 1, 2014 a payment for the $177.00 balance on the accountwas submitted by mail.A financial consent explaining our procedure for delinquent accounts.signed by [redacted] is on file. The expenses incurred when attempting tocollect delinquent accounts is passed on to the patient as noted in thefinancial consent.
