Complaint: [redacted] I am rejecting this response because:The business did not perform an dental care so the claim was disallowed by insuranceAdditionally I lost a day of work and had to schedule with a new provider capable of providing careI had to pay for the services twice In addition the tooth doctor erroneously reported a collections account which resulted in the denial of credit and an increase in my insurance rates and credit card interestAdditionally I have had my pacemaker since and seen multiple dentists and have NEVER been required to submit documentation from a specialist Additionally the Tooth Doctor did not provide detailed information that prior documentation from specialist was a requirement when they took my health information when I made the appointmentPlease reopen the case Regards, [redacted]

This is [redacted] from The The Tooth DoctorI am sending this email regarding Complaint ID: [redacted] Patient, [redacted] [redacted] chief complaint seems to be that Dr [redacted] would not treat the patient without consulting the patient's specialist due to lengthy medical history As mentioned earlier Dr [redacted] is not longer on staff at The Tooth Doctor Also she is not in the city of San Antonio I would like to get some type of testimony on her behalf regarding this issue I should have that in hand by Monday, April 13, What other documentation would you like from our office to help settle this matter? Also I see that the patient has stated that Disputed Amount of $1,has been mentioned I am not sure where this figure is coming fromIt would be greatly appreciated for a bit of clarification regarding that As stated by the patient all that we preformed on the date of service was Exam and X-rays The total of all procedures that were preformed that day, does not amount anything close to the Disputed Amount Thank you for understand Please feel free to contact the office at 210- [redacted] [redacted] The Tooth Doctor

I had to pay for the services twice.  In addition the tooth doctor erroneously reported a collections account which resulted in the denial of credit and an increase in my insurance rates and credit card interest. Additionally I have had my pacemaker since 1997 and seen multiple dentists and have NEVER been required to submit documentation from a specialist.  Additionally the Tooth Doctor did not provide detailed information that prior documentation from specialist was a requirement when they took my health information when I made the appointment. Please reopen the case.

[redacted] chief complaint seems to be that Dr. [redacted] would not treat the patient without consulting the patient's specialist due to lengthy medical history.     As mentioned earlier Dr. [redacted] is not longer on staff at The Tooth Doctor.  Also she is not in the city of San Antonio.  I would like to get some type of testimony on her behalf regarding this issue.  I should have that in hand by Monday, April 13, 2015.    What other documentation would you like from our office to help settle this matter?  Also I see that the patient has stated that Disputed Amount of $1,000.00 has been mentioned.  I am not sure where this figure is coming from.
