To Whom It May Concern:I had 4 Boost Mobile Referrals of friends who live in [redacted] was an [redacted] user looking for new carrier providerWhen I herd there where shrinking payments every 6 months Plus a 100 dollars off with a new cell phone with carrier exchange programOn Nov 2nd 2013 I decided to become a Boost MemberI paid the Following: 100 dollars for a Samsung Prevail Cell Phone at a Boost Mobile Store. 55 Dollars For a 1 Months of Service 5 Dollars For 100% Insurance CoverageWhile driving to North of Durham my phone calls dropped off and while being at work as wellI service medical life support equipment on the road for my hospital a service that is always neededProduct_Or_Service: Mobile Galaxy Prevail 2Order_Number: XXX-XXX-XXXXAccount_Number: XXXXXXXXXXXXXXXXXXDesired Settlement1. I am requesting for Boost Mobile to refund the $55.00 + the $5.00 Insurance I paid I have no issue if the dollar amount is prorated2.
