I am rejecting this response because:The billing associate that I talked to was extremely rude to meI tried to explain that we were never told there were additional cost with the tests that were performed at the end of the pregnancyWe were told we had paid all of the bills for this pregnancyWe should have been told when they wanted to perform these new tests that they were outside of the billing structure and that we would be charged additional for these We made sure that we had paid all of the bills in advance so we wouldn't have to pay any other billsWe had not budgeted for these additional cost eitherWe had given them our insurance card so they could see all of the cost that we would have to pay based on out insurance coverage If they knew we were going to have to do these additional tests they should have included them in the cost they had us pay in advance

To Whom it May ConcernWhen a patient comes in for her first OB visit we obtain her insurance card and information, we then contact her insurance and get her effective date and benefits, then we mail out a new OB letter that I have attached (I have removed any personal information) This letter
explains her benefits and what her pay plan will be. In this letter it states that this is an estimate, there is also costs that are NOT INCLUDED in the global fee and will be billed in addition The non-stress test that this patient has question is listed there as not included in the global. We also explain what the global includes. I understand that this was the patients fifth child and has never been a high risk before but we do follow the UpToDate guidelines for Advanced Maternal Age (AMA), If a woman is over the age of 35, she is considered to be of AMA, and with this title comes risks for mom and baby. Our doctors give the best care possible to make sure there is a good outcome for the patient and her child. We did bill this patients insurance for the non-stress test with the diagnosis code for AMA and her insurance did cover the claim. Because the patient has a deductible that needs to be met by the patient before the other benefit kicks in and is paid at 100% there is a portion that the patient did owe after we took the insurance write-off prour contract with the insurance My billing staff member did try to explain this to the patients husband even thou he was not polite. We do try to give the best possible care and try to help the patients understand their benefits and risks. I know that the additional tests would not have been done unless they were needed.I am not sure that the attachment came in like it should have. If I need to send it in the letter instead of attaching it I will be happy to respond. I do want the letter included because it is the letter we sent this patient to help explain this exact issue at the beginning of her pregnancy

To Whom it May Concern:This patient did have an appointment with Valley Obstetrics and Gynecology on December 12, 2015 at our Saratoga Springs Office.  She called our office and  rescheduled her appointment because she had a conflict with her scheduling.  We then gave her a new...

appointment for December 29, 2015 in our American Fork Office because on December 29th our Saratoga Springs Office was closed and not being used.  This would explain why the doors at the Saratoga Springs Office were locked and not open.  This patient said she checked her reminder text that she received and in that text it says the location of the appointment as being American Fork Office.  The patient should be familiar with this location because she has had appointments there in the past.  She then said she called our office to check on the correct location and got our answering service at 4:00 PM.  Our phones are on until 4:30 PM everyday except we did close early on Christmas Eve December 24, 2015 and New Years Eve December 31, 2015.  We had other doctors  an CNM's that had appointments on December 29, 2015 that was in the American Fork Office with the doors unlocked until after 5:00 PM.  When a patient does not show up for an appointment we do make it as a "No Show" for our records.  The patient said she called the next day to get an appointment and yes I'm sure we were booked in solid because of the end of the year and others who wanted to be seen in 2015 for insurance reasons.  We do try to work with patients and get them in when we can, but we do have to leave spaces for emergency visits too.  This patient did reschedule her appointment for January 7, 2016 at the Lehi Office. We then sent a reminder text to this patient with the date, time, and location like we always do. I don't see where this was a scheduling mistake on our part. The patient knew when the new appointment was made that she would see our doctor in 2016 and that there was going to be a charge for the visit.  We do know that having different locations of our offices can get confusing, but we do make every effort to let the patients know the location of their appointment and also include it with a reminder text for each visit.  The patient was the one that rescheduled her appointment from December 12, 2015 to December 29, 2015 and possibly got confused on the location.  She was aware of our different locations because she has been seen at all three of the ones the we have mentioned.  I do know that the phones were on at the time she said she called our office.  The patient did receive services, her insurance has responded and there is a balance due from the patient.

My wife has dual coverage insurance. The billing department has failed since January to properly bill the insurance companies.
My wife, [redacted], visited Valley Obstetrics and Gynecology on 1/28/2013. She gave them my insurance through SelectHealth which for her is secondary insurance but forgot to give them the primary insurance which is through Community First/First Health. We got a bill a couple of weeks later in February, she realized her mistake and she gave them the information for the primary insurance. She has made this mistake at other doctors offices but the situation was rectified within a month.

The billing woman, [redacted], at Valley Obstetrics and Gynecology kept putting my insurance as primary when it was not. She has continued to fight my wife that SelectHealth is the primary insurance. It is not. We do not decide which is primary or secondary, the insurance companies do. Community First/First Health is primary insurance for [redacted] and Select Health was secondary. Since then I have accepted a new position and so Community First/First Health is still her primary insurance but Regence Blue Cross Blue Shield is now her secondary insurance.

In July I contacted [redacted] of Northwestern Mutual who was the insurance broker for my former company where I had Select Health. She has assisted me in getting this situation corrected. I received an Explanation of Benefits from SelectHealth that was processed on 8/2/2013 showing that all charges have been paid. The line item which I received the bill full shows as $0 for patient responsibility. I will gladly send this document to you.

From all accounts this account has been paid by insurance. We are tired of dealing with [redacted] and will no longer be using Valley Obstetrics and Gynecology solely because of her. The receptionist, nurses and doctors all did their job professionally. It is a shame that the billing department is causing these practitioners to lose a customer. We hate to switch doctors mid pregnancy but we see no other choice if we cannot trust the billing practices of the company.

I did leave a firm voice message tonight for them informing them that we would not pay the $150 as it is covered by insurance and that [redacted] would no longer be going to this facility.
