submitted within 10 days of your visit. This is actually very quickly per billing benchmarks. As of today, we are still waiting for your insurance to process. Insurance companies can take between 45 and 60 days to process and pay claims, on the average, if everything is coded and submitted correctly. Prior to your visit we contacted your insurance company to get their estimate of your portion of the bill. We do ask for that money at the time of your visit, another industry standard, to spite any information that your insurance company may provide you to the contrary. That is our policy. Patients are notified when appointments are scheduled that they should expect to pay their copays and coinsurance at the time of the visit. Should there be any discrepancy in the amount that you paid and the amount that you owe, refunds are ALWAYS issued.

I had not yet received their refund when I submitted my previous response. Anyone who reads the entire exchange can see that I was simply requesting what was due to me.
I have now received their refund, but how they have responded shows their arrogance. They only followed the required procedure after I submitted this claim. It is too much of a coincidence otherwise.
Any reasonable person is strongly advised to seek medical help elsewhere. I certainly have found quite easily another surgeon, who by the way did not ask for my money in advance, since my insurance does NOT require it.
