Thank you for the opportunity to respond to this claim . The individual stating the claim was seen on 8/2011 for anterior esthetic work on her front teeth.  After the treatment was finished instructions for proper care were given and the patient was informed that other treatment needed to be...

performed on other teeth including regular check-ups. The patient did not return until 9/12 ,almost exactly a year later. A cleaning and exam were performed and suggestions for treatment were again given.The patient never complained about the previous work but was concerned about other areas of decay. Another appointment was scheduled for a molar on the lower left side to be addressed. That dental issue was addressed on 10/12 to the patient’s satisfaction. Again home care instructions and recommendations were given for other problem areas. The patient returned again in 2/13 complaining of another issue concerning another tooth not yet treated. Again recommendations were given to the patient. The patient did not return for that specific issue. Again there were no complaints about the previous work performed. In 11/2013 the patient came in complaining of a separate dental issue not related to any of the previous visits. Again recommendations were given . Patient did not schedule for treatment. Recently the patient recently requested a refund for treatment performed on the lower left molar treated in 10/12 . The refund was processed  with our objection. Now the patient is seeking a refund for the anterior work performed in 2011 complaining that the restoration fractured and that she should have a guarantee. The patient has been informed that there was no initial guarantee on the work. The patient never followed up with our suggestions for maintenance and only come in sporadically to address issues not related to the previous work performed.  The patient has accused this office of faulty work when she herself has not addressed the multiple dental issues that she has. This patient never complained about any previous work and now is seeking restitution for a situation that was never addressed . Please feel free to contact us at any time if needed.

Review: I paid $2525 dollars for two veneers in 2011. I paid $1427.00 in cash and $1098.00 on a credit card. My veneers have not lasted for the duration of the 5-10 years I was quoted and it contacted Dr [redacted] asking for an appointment to come to his office so that he could repair or replace the veneers. Dr. [redacted] refused to have me as a patient and told me he was not willing to help. I called my credit card company and they did an investigation, they deemed Dr. [redacted] at fault and did charged back for the$1098.00. I then called Dr. [redacted] again asking for a reimbursement for the amount I paid in cash and he started swearing and yelling at me, then ended the conversation by disconnecting the call. He also threatened me to stop perusing this or I would be sorry.Desired Settlement: An apology and a refund for the remaining $1427.00
