Business was repeatedly asked that charges were not over [redacted] yearly limit, but went over immediately by over [redacted]. teeth not fixed. Total sham.
This letter I sent to the dentists & have gotten no response:
Dear Drs. [redacted] and [redacted]
I am writing this letter to you to hopefully adjust my son's bill appropriately, and resolve this issue. I do not want to report to Indiana Insurance. His name is [redacted] Account # 4922.
[redacted] has been away for a couple years, and was in need of a good teeth cleaning. He has never had cavities, so I actually chose your practice from a Valpo flyer with a free whitening coupon upon new patient exam and films.
I called and spoke to TARA March 2013, she took all his information to check insurance, and we made appointment. I told her at that time our policy has [redacted] deductible when diagnostic, and pays at 80%. I knew my benefit had a limit of [redacted] per year and told her that-especially since [redacted] was not employed.
She called me prior to appointment to state the above benefit back to me, so he would know what to bring with him. (She stated she spoke to [redacted] at our insurance & that is our benefit person) We-meaning TARA did not know if deductible would be applied for 1st appointment or not, but we paid [redacted] 3/27/13 anyway, for future 20% co insurance.
TARA called me a few other times after 1st appointment, but before [redacted]'s next appointment (closely resembling harassment) to make sure I was sending money with him, even though claim had not gone to insurance yet, and she stated the [redacted] was not for 1st appointment-preventative. EVERY SINGLE TIME I SPOKE TO HER I VERIFIED THAT THE OFFICE WAS NOT GOING OVER THE [redacted] LIMIT. She said no.
I paid more co insurance the following week 4/3/13 35.00. Suddenly, no more calls, so I thought she finally got insurance payment and realized she did not have to harass us. Balance at that point was I think [redacted] had last appointment 5/29/13, and I called and spoke to [redacted] a new person prior to appointment again to verify office was not going over the yearly limit. I believed a dental staff to know about DENTAL INSURANCE YEARLY LIMITS.
He was unclear or unsure about the insurance, but would check on it. [redacted] had a total of 4 appointments, and was charged way above the limit.
I'm not sure he even had cavities-he had no tooth aches or pain prior. How are we to know?
He never got his "free whitening"
His teeth are not done yet, and do not look cleaned.
He just had one filling fall out last week, which has been "poking/scraping" since it was done 5/29. He can not have any care now until January 2014-and you are ok with that, and how you left his teeth?

My co insurance due should be 20% of [redacted] because the office chose to go over-either using the most expensive materials known to the industry, or because they just ignored the request of the customer. REPEATEDLY!

He should have had his teeth cleaned like was asked. My husband's dentist knows the benefits, and does some work this year, and the rest in January if needed-and that is on all patients.

I have paid [redacted] deductible, and a total of [redacted] co insurance, so correct balance is [redacted] 20% of [redacted] = [redacted]

I am a biller in the medical industry, and I will tell you if a patient calls me with this request I take steps to uphold the patient's request. Why would I incur an outstanding debt for my practice? Especially knowing they are unemployed.
