Dr. Mariani's office owes me a [redacted] refund since he was paid by me, my primary [redacted] of ** & my secondary dental ins. [redacted]I received dental services from [redacted] 2014 through [redacted] 2014. by Dr. Mariani's practice. The office personnel required me to pay [redacted] as follows: On [redacted] 7, 2014, before I had some work done on my gums, I paid [redacted] On [redacted] 2014 they asked me to pay [redacted] after I had surgery (4 teeth were pulled) and was still under the effects of medication. I have dental insurance coverage from my primary insurance provider, [redacted] of ** and by my secondary insurance provider, [redacted] I made sure that Dr. Mariani was an in-network provider with [redacted] & [redacted] before I went to the office. The total contract amount for all services equalled [redacted] according to the Explanation of Benefit statements from [redacted] my primary insurance. The amount that Delta Dental paid on my behalf equalled [redacted] According to the [redacted] explanation of benefits statements, they paid [redacted] on my behalf. However, they paid as a primary instead of a secondary provider. [redacted] responsibility as a secondary provider is [redacted] Since the contracted amount was [redacted] the primary ins responsibility was [redacted] and the secondary insurance responsibility was [redacted] my responsibility was only [redacted] I paid [redacted] so Dr. Mariani's office owes me a refund of [redacted] I first contacted them on [redacted] 2014 and faxed the Explanation of Benefit statements I received from [redacted] I've called them every month since then. I spoke to [redacted] in Dr. Mariani's office and faxed copies of the [redacted] explanation of benefits many times. The last time I called them was [redacted] 2015, at that time, I was told that [redacted] no longer works there and the office changed their name.Desired SettlementI am seeking a refund for [redacted] that was overpaid.
