I am a year old, semi-retired, salaried, independent contractor dentist, working three days per week for the [redacted] *** The complainant was a patient of record at [redacted] office and her attending dentist of record was Dr [redacted] *** I first met the complainant on August 11, when she came to the office on my watch as an emergency visit complaining of pain in tooth # My examination revealed extensive decay into the nerve and I referred her to the specialists at [redacted] for root canal therapy At this time the complainant was years old and she came to the office with her mother The complainant requested pain medication until she could get her root canal treated, and I was informed by her mother that the complainant was recovering from drug addiction and that she was not allowed to receive any pain medications The drug addiction explained the rampant dental neglect noted on her first visit by Dr*** The complainant continued her treatment with Dr***, and Dr [redacted] constructed a fixed, porcelain fused to metal bridge from tooth #to tooth # After the bridge was cemented, the complainant decided that she was no longer happy with Dr***'s bridge, and after complaining to the [redacted] management, it was decided that I should remake Dr***'s bridge, at no charge to the patient, since I was the senior dentist with the most years of experience I took over as the complainant 's attending dentist and I also constructed a fixed, porcelain fused to metal bridge for The complainant from tooth #to tooth #and a valplast, partial lower denture to replace the teeth missing in the lower jaw I first became aware that the complainant was unhappy with my bridge #to #on November 18, She wanted the bridge remade simply because another dentist told her it should be remade We took new radiographs and intraoral photographs.My crown prep technique for PFM crowns involves a knife edge finishing line all the way around the tooth No shoulders or champers I double checked all the margins and none were open Soft tissue around the bridge was pink and healthy with the exception of the gingiva around tooth #15, which had an existing all gold crown done elsewhere before the complainant became a patient of this office At this time we happened to have another of [redacted] dentists in the office, Dr***, and I asked her to evaluate the complainant 's bridges as a second opinion Dr [redacted] could not find anything wrong with my bridges eitherOn January 18, the complainant returned to the office demanding fillings on teeth #6, & # I examined the teeth in question and did not find any decay, presentTooth #had some gingival recession and toothbrush abrasion at the gum line and since there was no sensitivity, I advised leaving it alone, because drilling into the not decayed root could very easily result in sensitivity after the procedure The complainant did not return to our office for another months until she developed pain in tooth #consistent with pulpal necrosis I informed the patient that sometimes the nerve in a tooth dies after a major restorative procedure and I referred her back to the endodontist at Apex Endo to do a root canal therapy through her existing bridge crown This was never accomplished to my knowledge The next time the complainant was seen at the [redacted] office, she was seen on my colleague, Dr [redacted] watch Dr [redacted] examined both of my bridges in great detail and found them both to be within limits It is my opinion that the complainant's main complaint is that the bridges trap food and since she refuses to practice good oral hygiene she will get periodic infections in the gums as well as a bad taste and odor from around the bridges In all the visits that she made to our office she never once came in for routine recall visits and cleanings Finally, The complainant never once asked us for a refund of money spent Every time The complainant complained about the bridges she wanted them re-done And every time I then explained to the complainant that new bridges would turn out exactly like the current ones and she would still be unhappyThe complainant filed a complaint with the [redacted] regarding these matters and the board ruled against any remaking or refunding monies for these bridges.A little over a year ago the [redacted] was sold to another private dentist and is no longer part of the [redacted] *** Part of that sales agreement states that all [redacted] Family Dental patient's, including the complainant, would no longer be seen by any [redacted] Dental employees and are now the property of the new [redacted] Family Dentistry.Sincerely, [redacted] J [redacted] D.D.S General Dentist - Representing [redacted] - Camelback Family Dentistry LLC

I went to this dentist and he pulled 7of my teeth it was very painful I kept trying to tell him but all he said was I CANT UNDERSTAND YOU SO NO USE IN YOU TRYING TO TALK I acted if I was trying to write so maybe give me pen n paper again I was ignored from him and staff the receptionist completely Quoted me on getting dentures So I signed the agreement that I was paying cash for dentures and insurance coverage was exstracting 10teeth even though only 7was so far then the next day she calls me and says that since ins covred the exstracting n I was paying cash the payment went UP FROM$1,100 TO 1,500 thats so wrong of them to do that and to bill my insurace inwhich she said they are for work they dodnt do yet as in pulling my other4 teeth I might call channel 3 news to investigate them I am supposed to go back to pull teeth and get stitches out yes I have 6 stitches in my mouth he didnt just normally pull my teeth he cut the gums of each n did it I am still in pain and my face n eye stll swollen to 5 daus later to
