Hidden charges - Not fare - HEALTH !!! (no car insurance)
I went to see Dr *** *** at Mount *** on 7/and when there I asked to pay as molina healthcare of FL ask for a specialis visit on my card and on page of the agreedment with none Limitations & Exceptions and the lady on the front desk said that I do not have to pay (I thought that same happened with primary care for the first time visit = no charge) but latter they send me a an Invoice for usd(I went twice to Dr *** because of the results of the 1st visit)I called all (M *** invoice department / Dr *** manager-supervisor / Molina / and Obama Health Care and no-one take care of this imnportant situation about not tell me CLAER..FIRST how much it going to be before attend me as I asked allways

Authorization completed by my physician for fibromyalgia blood test Have not received authorization for medically necessary test as of today
My physician completed an authorization form for me to have a fibromyalgia blood test done and paid for by my insurance since it's medically necessary for them to determine the disorder and treat the disability I spoke to a Molina representative who stated I needed my PCP to fill out the form and the facility *** *** would submit the authorization form when they billed for the test I followed the steps as instructed by Molina and in turn I received a call from *** *** stating that Molina refused to pay for the test since *** *** is out of network I contacted Molina again and they stated the authorization needed to be sent directly to them I contacted my PCP, Dr*** *** and they tried faxing the authorization over times to the fax number X-XXX-XXX-XXXX All faxes didn't go through and the email confirmatio
