Regards,

Review: On or around 07/23/2013, I the complainant visited Simon Eye for an eye exam, prescription glasses and contact lens. During the period of my exam I was told by the doctor on duty that due to pressure in my right eye further testing would need to be done and that it required a referral from my primary physician. While in the office; [redacted]; my primary physician was notified and the doctor on duty was given the referral information requested and proceeded to perform the testing required. After I had picked up my contact lens / glasses and paying the required balance not covered by my insurance carrier, I started receiving bills regarding the testing in the amount of $138.00. When I questioned Simon Eye Associates of the bill; they stated that the referral was not acquired and that I was responsible for the balance. After clarifying that the referral was obtained via land line from their office during my visit, they change the course of the billing stating; that it was for the balance of my glasses, which I knew to be a lie; because I had paid the full receipt balance of my glasses and my contact lens was covered by insurance. And knowing Simon Eye Associates like I do, you will never walk out of their office with a product that has not been paid in full. I re-visited Simon Eye Associates (both offices) her in New Castle County and was told by the billing receptionist that she would contact that the Main Office and have the problem adjusted. No communication was received from Simon Eye Associates for about 2 - 3 months; then I received a bill from [redacted], after speaking with [redacted] Services I then re-visited the office and was initially given a B.S. story until speaking again with the billing personnel who stated to me that she was investigate the matter and due to the low billing balance have the billing department clear the balance. Well during my waiting period I am now being billed by the same companies again for a service rendered that I was told was initially covered by a referral. Now I have not been back to Simon Eye Associates for their services after this unfortunate event and plan to never return. I feel that this was an in-justice on their part and I am being billed and penalized unfairly after taking the medical advice of their staff. No other eye associate that I have visited in Delaware or Pennsylvania have I encountered this professionalism. And today it continues. Simon Eye Account [redacted]Desired Settlement: I would also request a letter of apology from the company for the unnecessary stress and collections actions that they have taken against me; I have been in both of their offices in attempt to rectify this matter and we continue in the same circle of nonsense.

Review: The problem occured on Saturday, November 22, 2014 when my son was scheduled to have an appointment in the morning. Now my son has always had his appts at the Fox Run office however the rep who made the appointment failed to advise me that the appt she set for me on that saturday was for the limestone office so I arrived at the fox run office and was advised that I was at the wrong office and the doctor could not fit my child in until later that afternoon and that I could come back I asked the receptionist what would happen for my inconvenience and she advised me that she would have it looked into so I left the office without making another appt. I then received a call the week of the 24th asking if I wanted to schedule another appt for 11.29.14 at 10:30am at the office in wilmington on route 202 so I accepted that appt for my son we went and he had a vision exam done where it was determined that his lense in the left eye would need to increase my son has a deformity in his right eye that causes him to have 20/1300 vision so with the type of vision insurance I have he is alotted 120.00 towards frames and lenses and then a discount off the remaining balance as well so while we are shopping for some frames the rep advised me that his alottment for the year was used in march I advised her that I have only ever brought him a new pair of glasses in september every year she then advises me that per the notes in the system I paid the difference for a pair of glasses for him in march I said to her well if that's the case the person at the fox run office never explained that to me and if that's the case he should have been allowed to choose a different style of frame she advised me she would investigate on monday and call me back she called me on 12.02.14 advising that she spoke to the fox run office who advised her that I used his alottment to purchase the glasses in march because of the dog scratching the glasses I advised her that the dog scratched up my glasses not andre's and their notes were incorrect she offered me to upgrade his lenses in the same frames that he already has and then he could get a new pair of glasses in january I advised her that her organization has messed up on several ocassions and this is a case that they should fix she said that they are trying to help me by giving my son the new prescription for his glasses and then he can get the new frames in january to me this is a major inconvenience I told her and advised her I would be contacting the Revdex.comDesired Settlement: I would like for the fox run office or whoever made the error to contact my vision provider and advise that they filed an incorrect claim in march because it should have been issued as a replacement not a new pair of glasses and allow my son to obtain his glasses that he was excited about getting this month and not have to wait until january which is another inconvenience from Simone Eye
