Company Dear Ms***: This complaint was forwarded to me for a responseForethought Life Insurance Company (“Forethought”) issued policy *** to *** *** on or about 06/16/Sadly, Mrs. *** passed on 12/12/Forethought received notification of the claim on 12/30/2016. Since she passed within two (2) years of issue, the policy is contestable and was assigned to Donna B*** (“Donna”)A contestable claim means that Forethought has the contractual right to contest the validity of the policy by verifying that Ms*** truthfully answered the underwriting questions on the application and fully disclosed all material information about her health historyVerification is achieved by a review of the deceased’s health history as recorded in his or her medical records. Accordingly, Donna sent an Authorization to Obtain Information to *** *** on 12/30/This form permits applicable health care providers to release records for *** *** to Forethought so that we may conduct our investigation and determine the amount payable for the claimDonna received a copy of the authorization, signed by Ms*** on 01/09/Since she was not the correct person to sign the authorization, she resent the request to *** ***. On the authorization she provided, Ms*** identified Dr*** *** as the primary care physicianThe authorization also listed *** ** *** and Dr***, a specialist; however, the records from those providers are not germane to the investigationIt was our belief that Dr*** was affiliated with *** *** ***. On 01/09/16, Donna sent a request for medical records to *** *** ***/Dr*** using the authorization provided by Ms***Donna received a copy of the authorization signed by *** *** on 02/01/and sent that authorization with her second request for records on 02/13/Donna sent a final request to for medical records on 03/13/17. She also sent a letter to Mr*** inviting him to contact *** *** *** on our behalf to assist with obtaining records. Ms***’s complaint also noted difficulty in contacting both Forethought and Donna. She mentions that when prompted by the phone system to enter a policy number, she received a message that our office was closed but if she did not, she was able to speak to a representative. Our policy owner phone line closes at 5:PM EST and any call to that line after that time would receive the message that our office is closedHowever, we do have customer service representatives staffing phone lines until 7:PM EST to, among other things, answer calls related to death claimsFinally, the system notes document only one call from Ms***, which occurred on 03/24/and was answered by a customer service representative named TrishaDonna was out of the office that day and has no record of a call or message from Ms***. We have called Ms*** in an attempt to help resolve the complaintAs a result of the call, we have learned that Dr*** is affiliated with *** ***We have obtained their fax number and today (03/29/2017) faxed the authorization and medical records request to *** ***Once we receive the requested records, we will complete our investigation and make the appropriate benefit payment.
