May 9, 2014 To Whom it May Concern My name is [redacted] ***, MD and l am the Medical Director for 1513 Anti-aging and Weight Loss Clinic. This letter is in response to Case #l [redacted] 7. All medications prescribed by my Physician Assistant, [redacted] or myself are reviewed and only administered when clinically indicated. if a patient displays contraindications to medications such as Phentermine, clinical decisions are made in the best interest of the patient. We at 1513 weigh the risk vs the benefits before prescribing medications. Our current weight loss patient information sheet clearly states that appetite suppressants are included in the plan if appropriate and prescribed by the clinician.  The doctor was never present at any of my visits and they definitely never gave me a sheet stating that I wouldn't get the appetite suppressants because of contraindications. I was offered the suppressants the day I was given my shakes by the PA and she also told me that most patients don't need them. The business is new and apparently not wanting to fully uphold all that is promised. I'm a senior enlisted member in the [redacted] and will definitely be reporting this business to the Garrison Commander to add this business to the black list so that it will be off limits to Soldiers.
