This policy was issued new on 5/5/The insured's 3rd installment of $was returnedfrom her bank for insufficient funds on 7/14/The policy cancelled on 7/29/The insuredmade an online payment for $and the underwriter agreed to reinstate the policyTheagent requested that the policy be set up on Electronic Funds with new bank informationThepolicy reinstated.The mistake on ACUITYs part came when policy was reinstated and set up on EFTThe policywas behind in payments and the incorrect number of installments were set upFourwithdrawals were attempted from the insured's bankOne of the four was returned as anunsuccessful paymentWe were given documentation showing were successfulWe thenreturned of the three to the insured's account to correct the mistakeThe insured and agentwere notified of the situationACUITY kept one of the installments as one was dueAfter wereturned the $(of the payments) we were notified that the insured had stopped payon those installments that we returned.The insured lapsed for non-paySince the insured placed a stop payment on the two paymentsrequested in error, the refund for the two payments is due back to ACUITY.Please find the attached documentation that we received from the insured showing thepayments and the summary showing that we returned these paymentsThis was prior to herstopping payment on the paymentsThe agent requested a new bank statement to verify thatshe received the refunds from ACUITY and she refused.We are now billing the insured for the return of the refundThe insured had insurance 5/6/to10/1/Please let us know if we should send a copy of explanation to insured again
