In most cases, co-pay is a small amount (under $50) and annual deductible can range from $50 to $250 or higher per person per year. We are a participating in-network provider under this specific health insurance plan. At the time of the visit, the office staff correctly collected the "co-pay" amount. Our billing staff then sends a bill to the patient's health insurance company. The insurance company delineates what is allowable charge for any service and what is patient's responsibility under annual deductible. After a payment was received from the insurance company, our billing staff sent a bill to the patient only for the amount that the patient is responsible for.
