Review: To whom it may concern;

I underwent extensive medical treatments for breast cancer in late 2009 and received bills from

at least 12 different medical groups. One of these was ADL, Inc ([redacted]

Anchorage, AK XXXXX-XXXX; XXX-XXX-XXXX). I have copies of all the bills I received from ADL

and will attach copies to this complaint if possible. The bills were incredibly hard to make sense of, I

received eight bills between 9/14/2009 and 1/4/2010 and although there were only two account

numbers listed [redacted] and later with letters & numbers reversed QPCIX-XXXXX) the bills

had widely varying amounts owed with overlapping charges (bill1:9/14/2009=$189.00;

bill2:10/15/2009=$239.25; bill3:10/21/2009=$189.00; bill4:11/16/2009=$522.75;

bill5:11/23/2009=$50.25(past due amount); bill6: 12/23/2009=$283.50(past due amount);

bill7:12/30/2009=$50.25(final notice); bill8:1/5/2010=$144.75

It is clear from the bill details that there were overlapping charges on each bill so it was

impossible for me to tell how much I owed exactly. I contacted ADL to request

clarification on the highly variable amounts owed on each bill and in response to Final Notices I

did pay $189.00 ([redacted] Check No. [redacted] dated 12/29/2009) and $143.10

([redacted] Check No. [redacted] dated 2/22/2010). Based on the bills provided by

ADL with these payments I believed I had settled this account. I did not receive any further

correspondence from ADL and never received notification that the account had been sent to a

collection agency.

In February 2014, I was shocked to see three accounts listed on my credit report as in collection

with[redacted] (PO Box XXXXX Anchorage, AK XXXXX; XXX-XXX-XXXX) identifying ADL as the original account. To my

knowledge I have never been contacted by[redacted].

Since I believed I had paid ADL, I sat down with an excel worksheet and attempted to figure out how I could still owe money based on my records. I was upset to calculate that I might owe money but shockingly not for the amounts/charges cited on the credit reports.

Date of Service Amount Owed

8/27/2009 $63.30 (pd [redacted]

8/27/2009 $64.05 (pd [redacted]

8/27/2009 $61.65 (pd [redacted]

9/9/2009 $50.25

9/11/2009 $63.30

9/11/2009 $45.30

9/11/2009 $31.80

9/11/2009 $63.3 (pd [redacted]

9/11/2009 $39.9 (pd [redacted]

9/11/2009 $39.9 (pd [redacted]

9/11/2009 $1.65

In paying $189.00 (check No [redacted] on 12/29/2009-charges $63.30+$64.05+$61.65), I believe I paid the sums referenced in the remarks on the credit reports([redacted] account numbers XXXXXXX for original amount $63; XXXXXXX for original amount $64; XXXXXXX for original amount $61). In paying $143.10 (check No [redacted] on

2/22/2010) I paid three additional charges ($63.30+$39.90+$39.90). According to my accounting

I still might owe on other charges ($50.25+$63.30+$45.30+$31.80+$1.65 = $192.30). Although the sums did not match the accounts recorded by[redacted] listed on my credit reports, since the total amount was similar (but not exact) and since it appeared I might still owe money, on 2/21/2014 I called[redacted] and agreed to pay hoping to get these charges removed from my credit reports as soon as possible. Initially I was told that if I paid $250.00 (which would

pay the balance in full on all three accounts) then the negative information would be removed from my credit reports by 2/28/2014. When the information was not removed by that date I called[redacted] and was told it would not be removed from my credit reports but only updated as paid in full and that this process can take 3-12 weeks.

Since I now believe I have evidence I paid the charges currently listed on my credit reports with check no 911 I have decided to dispute the accuracy of the information and ask both[redacted] and ADL to provide full documentation showing why I still owed $63, $64, and $61 after paying those amounts in 2009. The amounts match as originally billed. The original bills issued by ADL were impossible

to make sense of but based on the information ADL provided me in late 2009-early 2010 I

believed I had paid in full with my two checks totaling $332.10. Now I have provided

[redacted] with additional funds above and beyond what I believed I originally owed. I

am writing to request the following actions:

1) I am requesting ADL and/or [redacted] provide me with evidence that I

did not pay the $63.30, $64.05 and $61.65 charges appearing on my credit reports as

in collection with check number [redacted] on 12/29/2009 for $189.00. If this check did not pay for these charges provide me with details of the exact services they did pay for.

2) I request that this information be removed immediately from my credit reports. It is

my view that ADL and[redacted] are not reporting accurate information on my credit reports in citing these specific amounts in collection.

3) Furthermore I request that[redacted] refund my $250.00 or

provide me with the exact original ADL services/charges that this amount would

cover. I believe that the original bills were so hard to interpret that I should not be held responsible for any unpaid amount. I believe that any impartial arbitrator reviewing the correspondence and subsequent actions taken by both [redacted] and ADL would agree with this conclusion.

If these two companies cannot make sense of the original amounts owed and which were paid

that would explain why I have had such a difficult time trying to determine what or if I still owed on these accounts. How can I be expected to do something that both ADL and[redacted] seem incapable of - determining the correct amount owed for the medical services provided and clearly communicating which services have been paid for and which have not. I believe that ADL made the original error in reporting these specific charges as unpaid due to their nonsensical billing process and[redacted] perpetuated the mistake by reporting this incorrect information to the credit reporting agencies. This complaint and documentation will be submitted to each credit reporting agency, the Revdex.com Alaska, and The Consumer Financial Protection Bureau filing against both companies, ADL and[redacted].

Sincerely, [redacted]

Desired Settlement: 1) I am requesting ADL and/or [redacted] Services provide me with evidence that I

did not pay the $63.30, $64.05 and $61.65 charges appearing on my credit reports as

in collection with check number [redacted] on 12/29/2009 for $189.00. If this check did not pay for these charges provide me with the

services they did pay for.

2) I request that this information be removed immediately from my credit reports. It is

my view that ADL and [redacted] Services are not reporting accurate

information on my credit reports in citing these specific amounts in collection.

I believe that the original bills from ADL were so hard to interpret that I should not be

held responsible for any unpaid amount. I believe that any impartial arbitrator

reviewing the correspondence and subsequent actions taken by both[redacted]

[redacted] and ADL would agree with this conclusion.

I can provide a pdf document of ADLs original bills to me (12 pages) along with copies of the checks paid to ADL along with the bills & sums paid for by these checks indicating the three accounts listed on the credit reports were paid for by check no [redacted] on 12/29/2009.
