Patient Name: [redacted] was in our office on 3/11/for the Dental TreatmentRestorations were treatment planned and patient was offered Amalagam (silver) filling and Composite (tooth colored) filling patient choose Composite (tooth colored) filling and also patient was informed by the office Manager that her Insurance does not covered the Composite (tooth colored) filling so she has to pay out of pocket and patient agreed to thatUpon completion of Restorations patient went to the check out window and front desk ask for the payment patient started arguing that she was never told about the payment that she has to make and patient left the office without paying a single penny towards the treatment.Thanks,
I have spoken to the Regional Manager for A-Family Dental and we reached an agreement of a zero balance in my favor but I would first like this in writingHe then forwarded it on to their attorneyI would still prefer a letter stating that I will receive a zero balance due to trust issues with this company The Attorney General is not trying to mediate this situationThey are aware of the situation and their letter does list what mutual agreement we have come to and I do not feel comfortable with that statement unless it is listed that I owe zero Revdex.com:I have reviewed the response made by the business in reference to complaint ID [redacted] , and find that this resolution is satisfactory to me Regards, [redacted]
