It is hard to respond to a complaint that has so much fictional information listed.  However, I will give you the facts that were gleaned from my staff at the time the complaint was addressed in house.  1)  Our receptionist, Meagan, made the initial contact and appointment.  At...

that time no mention was made that the voucher would expire prior to the examination date.  Therefore, no promise was made to honor an expired coupon.  2)  At the time that services were rendered, the client, Jordan D[redacted], DID NOT present any paperwork from the shelter, including the voucher until after she was in the examination room and  Dr. Katherine L., a relief doctor, had begun her initial examination.  The tech on duty, Tristen, stopped the doctor and asked for paper work.  She then informed the client that the voucher was expired and we would not get reimbursed for it.  However, we were willing to do a courtesy exam, if she paid for a  full set of vaccinations which the client agreed to.  After the exam was completed and the doctor went over the report card, the client THEN DECLINED additional services and attempted to leave the clinic WITHOUT paying for the exam.  Her MOTHER called later that day or the next (Wednesday, June 3rd) and asked for the office manager.  She was told that the Secretary of the Corp. would be in Monday and would address her concerns.  ON Monday, June 8th, I, Shirleen H[redacted], the secretary of the Corp. spoke extensively with the MOTHER.  At that time the mother was told to send a letter with her complaints to the office of the Corp., in care of the South Jordan location and attention to Shirleen H[redacted].  That letter was NEVER received!  In 27 years of working with the Humane Society of Utah we have NEVER had this situation come up and have a good working relationship with them.If you need any additional information I can be reached Mondays only at [redacted].Sincerely,Shirleen
