[To assist us in bringing this matter to a close, you must give us a reason why you are rejecting the responseIf no reason is received your complaint will be closed Administratively Resolved] Complaint: [redacted] I am rejecting this response because: The information Beckie R [redacted] has provided to you is false I was not release from my treatment on July 18, I still had few more appointments to goI decided to stop going to the treatments after my appointment on the 21st because the Doctor told me that he will need to treated as a new case but neither the Doctor or staff told me that the new case was going to start that same dayShe is also stating that I had enter the office on my own complaing about neck pain but if that was the case why they didn't charge me the same day? I did had an appointment on July 21st from my first car accident and since I was on a treatment I had to mention to them that during the weekend some drunk guy hit me on the back of the rental car Its very upseting that now they are just lying and covering the truth Regards, [redacted]

Re: Appropriate charges for services providedThis patient entered our office on 6/10/complaining of pain she was experiencing as a result of being involved in a motor vehicle accidentOn that day, along with filling out necessary paperwork, she was consulted with as to the description of the accident, when it happened, how it happened, was a police report issued, what were her complaints etc.She was a new patient to our practice and so was given a thorough orthopedic, neurological and chiropractic examination including several x-raysShe was given directions for home therapy and instructed to return the following day for a complete report of findings including a plan of care and to begin active care.She followed her prescribed plan of care fairly well and as appropriate progress was made she was released from care on 7/18/14.Three days later, on 7/21/14, she entered the office complaining of neck and head pain that she experienced as a result of being involved in another motor vehicle accident on 7/20/As was appropriate, she was again given paperwork to fill out about the details of this current accidentShe again was consulted with and examinedX-rays were unnecessary to be taken for this accidentAfter the examination she was treated utilizing moist heat, electrical stimulation, intermittant traction and spinal and extremity adjustmentsShe was instructed to return in two days for follcareShe never came back.On that day of 7/21/14, she was billed for an appropriate and medically necessary examination and treatmentThe charges were usual and customary for this area for the services providedWe sent her several bills and got no responseWe tried to call her several times but her phone would not accept incoming callsWhen we received no responses from her after over months we sent her a letter warning her that this account was well past due and if we did not get resolution we would have no alternative but to send her account for collectionThen we got an angry telephone response from her.We provided necessary and prudent care for this patient and charged appropriate fees for the servicesWe also made every effort with a great deal of patience to resolve the balance on her accountWe take pride in the quality of care and the genuine concern we have for our patients, which is why we have been in practice for over years without a single Revdex.com complaintAnd so we do expect to receive payment in full for the services provided.If you have any additional questions, please do not hesitate to contact our office.Sincerely,Beckie R***Office Manager

I'm sorry to hear that this matter continues to remain unsettledI hope that I can add some more information that may provide more clarity regarding caring for a patient who has been involved in more than one accident in a short period of time.As stated in a previous letter, and in agreement with the patient, she had been being treated actively at this office for injuries she received as a result of a motor vehicle accident that occurred on May 29th She had been improving as expectedShe had an appointment scheduled for July 21st 2014.When she arrived at our office on that date and we were made aware that she was involved in another accident the day before, she was given a form to fill out giving us information regarding what happened in this most recent accident and how she was feeling as a result of the accidentShe wrote on the form that she had neck pain and head pain that was worse as a result of the accident.| told her that since the pain was worse since this recent accident we would need to open a separate account, do an exam to document the findings and treat her on this new account until she returned to the condition she was in prior to this recent accidentShe did not have any objections to this planI also told her that from a legal point of view she would need to tell her attorney (who was representing her for the first accident) about this accident so he could discuss with her, and us, how he wanted to handle it.After discussing how we would handle her care for this recent accident, I proceeded to examine her and provide treatment to herI gave her directions to use ice at home and do stretching exercisesShe was directed to return in two days.After not hearing from her after she left the office I asked my staff to call her to schedule a follappointmentIt was my understanding that when they attempted to contact her, her phone would not receive incoming callsWhen we couldn't reach her, we contacted her attorney who stated that they would not represent her for the second case (we are not aware of their reasoning for not wanting to represent her)Since we were not able to communicate with her for several months and the attorney needed to settle the case for the first accident, she was released from further care at this office for the injuries which were caused by the first accident.We continued several times to contact her but were not able toThe charges for an examination, spinal adjustment, extremity adjustment, electrical stimulation therapy, intermittant traction and moist heat on that day was $She had made no payments on it when she was at our officeWe had been sending her monthly bills for several months with no response until we warned her in a letter that if she didn't pay this outstanding balance her account would be sent to an outside collection agencyThat's when my insurance coordinator told me she got a very angry phone call from this patient stating she would not pay this bill.Since this patient wrote down that her condition was made worse by this more recent accident, and there was no reason for me to think that she was not telling the truth, it would be unethical for me to make the first insurance company responsible for an injury that was made worse by the second accidentThat is why I told her that we would need to create a new account for the care of that day and future care until she got back to the condition that she was in prior to the second accident.Both myself and my staff have done all we could to both care for this patient and to get this matter resolvedWhen caring for our patients we make a point of having clear and open communication between our caregivers and our patientsWe also are always willing to reach out more than halfway to help our patientsWe would never want to end a relationship on a sour note and so what I would be willing to do to resolve this matter is to take half off the charges for the services of this day and she would therefore be responsible for only $162.50, if we received payment for this amount by August 5th If this amount is not received by this date the full amount of $would be due and her account would be immediately sent for collection along with additional collection agency fees.If you have any additional questions, please do not hesitate to contact our office.SincerelyGary AD*** D.C
