Complaint: ***I am rejecting this response because: their rebuttal is completely untruthfulI brought my three children in and refused to have them get fluoride however we were charged for full cleanings with fluoride and they falsely documented that they completed full cleanings on my children's teeth the doctor only did the examination at the end however his assistants documented they provided care they did not provide we were falsely charged and I will not allow my family or others to be overcharged and lied to I am hoping that the Revdex.com can help this stop so these Crooks do not harm other families the way they have harmed mineAlso they did share my hippa covered patient information with the Oregon board of dentistryI did not sign any waivers however all of my children's charts were given to the Oregon board without my knowledge so they lied about that to this business and especially manager Sue should be charged with fraud.Sincerely,*** ***

Ms *** brought her three children to our practice on 3/06/She had private dental insuranceWhen we give estimated of what they will cover, we did not know they pay off of a fee scheduleWhen it did not cover procedures at 100% she was upsetI have copies of what she signed and it states
this is only an estimate of what they coverThis was work that was completed and they do have a balance to clearalso we did not violate any HIPPA rules with out consentShe also lodged a complaint with the Oregon Board of Dentistry and the case was dismissed and thoroughly investigated, case # is ***
