Complaint: [redacted] I am rejecting this response because: I honestly don't know where to begin, or how to react too these allegationsI am actually shocked by the accusations towards my brotherYou must of become really upset,and or embarrassed of the lack of hygiene not given too my father for over daysMy father was without a bath for fathers day and his birthdayYou all left him in this conditionmy father felt dirty and smelled this is no way too treat a patient all the excuses your staff gave Ms [redacted] , or her boss Ms [redacted] does not justify for the lack of care! Or how you all made my father feel during this timeI always reached out too your agency/ personal, and questioned as too why they were not providing my father with a bath 1.I heard excuses ranging from My father does not qualify for the bathing Your agency does not have any one they can send 3.Your agency does not have anyone willing too go bathe my father 4.It is a courtesy too provide my father with a bath, 5.we are working on hiring someone we will send someone on Monday after they are hiredand then No one ever shows These excuses came from [redacted] and [redacted] on a direct phone line via speaker phone in front of my father, brother and my husband This is actually not the first time your services lacked as far as careThis should not be news too you Being left unbathed; left my dad feeling bad about himselfand he expressed this too you all as he also got on the phone as well as my brother and pretty much begged you all too send someone too bathe him AS far as Mr [redacted] since you want take make accusations How about the fact that your staff member [redacted] claimed too have given my father a bath during one of his visits on a FridayThis was actually during the time my father was without personal Hygiene for days straightWhen [redacted] told me this "that [redacted] had given my dad a bath" I immediately asked my brother and my father if this was trueI come to find out that this was indeed a lieMy father got on the telephone via speaker and told this too [redacted] and [redacted] As they asked my father to verify it and he did, all they could say was "we will look into it" I asked them why he lied about it and I wanted to know if he had submitted payment for this service.They could not answer me never did answer me this questionAnd never cared too call us and verify any of that I reached out too the V.Asocial worker, [redacted] about the problems we were having with your agencyI was actually pretty disappointed by the lack of helpthis is why I did not call you all backand the only time I did not do so, because I had actually reported all of information too higher up at the V.A It was actually not until the V.Anurse [redacted] *came back from vacation that she and I spoke about your lack of services your agency was giving my father and all the excuses and the condition of my father that she then stepped in and clarified all the excuses your agency gave us were not acceptable! She made contact and this is when you all finally sent someone to bathe my father on that same day I believeNO EXCUSE what so ever for the lack of care given too my father I want to mention that immediately after this report was sent to Revdex.com on my behalf that you all decided too discharge my father from your all's careAll of a sudden his wound was healed why didn't you all ever report my father feeling like he wanted to hurt himself, maybe its because this was only because he was left filthy by your agency and didn't help the situation but none of this was reported to the V.A my brother questioned this too your staffthey said they weren't going to report it so I did too the V.A.as I care Why wasn't it reported too my fathers V.ADrthat your staff member therapist [redacted] burned my fathers leg, groin area and back applying too much Bengay on different occasionsAll he "***" tried doing was rubbing it off with waterbut it still burned my fathers skin from over applying my father reported this too [redacted] and [redacted] and the superviserNothing was done! My father has a burn area on his back still that I saw myself this weekendI did not know this kept happening, it was mentioned once to me and now clarified As far as the report too APS that was submitted, was probably out of revenge due too that Health Agency being reported for Fraud by meNo need too discuss that any further too youThank you *If anything [redacted] became angry during his visit after I had reported him to your agency about the lying, that he had given my father a bath and that I had questioned payment! *As far as the therapy sessions are in question as there are no records! thank you for your time Please Note: There is no excuse for leaving my a senior disabled Vet in that condition when your agency had the means authorization too bathe himMy brother does the best he can Please don't stoop so low and lie about him in that mannerI my brother was in any way mistreating your staff i'm sure I would have received a call about itBut I never did because its bogus! I am rejecting this response because:

You must of become really upset,and or embarrassed of the lack of hygiene not given too my father for over 21 days. My father was without a bath for fathers day and his birthday. You all left him in this condition. my father felt dirty and smelled this is no way too treat a patient.

AS far as Mr. [redacted] since you want take make false accusations . How about the fact that your staff member [redacted] claimed too have given my father a bath during one of his visits on a Friday. This was actually during the time my father was without personal Hygiene for 21 days straight. When [redacted] told me this "that [redacted] had given my dad a bath" I immediately asked my brother and my father if this was true. I come to find out that this was indeed a lie. My father got on the telephone via speaker and told this too [redacted] and [redacted]. As they asked my father to verify it and he did, all they could say was "we will look into it"
I asked them why he lied about it and I wanted to know if he had submitted payment for this service.They could not answer me never did answer me this question. And never cared too call us and verify any of that.
I reached out too the V.A. social worker, [redacted] about the problems we were having with your agency. I was actually pretty disappointed by the lack of help. this is why I did not call you all back. and the only time I did not do so, because I had actually reported all of information too higher up at the V.A.
It was actually not until the V.A. nurse [redacted]. came back from vacation that she and I spoke about your lack of services your agency was giving my father and all the excuses and the condition of my father that she then stepped in and clarified all the excuses your agency gave us were not acceptable!
She made contact and this is when you all finally sent someone to bathe my father on that same day I believe. NO EXCUSE what so ever for the lack of care given too my father.
I want to mention that immediately after this report was sent to Revdex.com on my behalf that you all decided too discharge my father from your all's care. All of a sudden his wound was healed.
why didn't you all ever report my father feeling like he wanted to hurt himself, maybe its because this was only because he was left filthy by your agency and didn't help the situation.
but none of this was reported to the V.A.

Please don't stoop so low and lie about him in that manner. I my brother was in any way mistreating your staff i'm sure I would have received a call about it. But I never did because its bogus!

Re: Complaint [redacted]Patient was admitted to services on 4/14/2017 with from VA for assistance with ADLs, which includes bathing, services with home health aide and were provided on 4/18/2017, 4/20/2017, 4/25/2017, 4/27/2017, 5/3/2017, 5/11/2017, 5/15/2017, S/18/2017, 5/19/2017, 6/2/2017, 6/9/2017....

Patient was also treated by Physical Therapy (24 visits ) and Occupation Therapy (28 visits) each to assist with improvement in strength- Patient was also seen by skilled nurse three times weekly for a total of 39 visits for wound care to left great toe.On 6/13/2017 [redacted] RN received call from [redacted] (Occupational Therapist) regarding patient reporting to [redacted], patient's caregiver not feeding him, and has not eaten all day. Patient reported scheduled medications also had not been given and patient was concerned he had not received his daily insulin. During the required investigation according to Best Practice Home Health Guidelines, [redacted] RN, was not able co make contact with the patient's daughter, [redacted], but left message to call the agency. per A-Plus Policy and Procedures and Medicare guidelines, a complaint voiced must be reported Adult Protective Services. A report was then placed and during the investigation, A-Pius Family Care, LLC was notified by Adult Protective Services that the caregivers had two open and pending cases of abuse/neglect regarding the patient's level care in the home. [redacted] of APS [redacted], who was listed as the case manager for Adult Protective Services for both pending eases and was contacted after a report was initiated by A-Plus to Adult Services. On June 14, 2017, patient's son (with whom the patient lives with and the primary caregiver), became hostile towards the staff ([redacted] LVN, the nurse who had been assigned to give patient his bath) ever the report filed by A-Pius Home Health to both Adult Protective Services. and Veteran's Administration Clinic/ Dr. [redacted] RN, explained to the patient's son, that the agency is required by law to report any suspected abuse/neglect on elderly patients and if patient made an accusation that patient was not being cared for at home, the agency is required to report to the proper authorities. [redacted] RN, explained to the patient's son and patient that since the son was agitated at employee who was assigned to bathe patient, Skilled Nurse would not be available to bathe the patient at the time due to patient's caregivers behavior towards the Skilled Nurse. Skilled Nurse also did address the concern about patient's safety in the home due to poor living conditions to son. The patient's son continued to yell at Skilled Nurse. [redacted] RN, spoke directly to patient and offered the patient an option of calling Veteran's Administration Clinic/Dr. [redacted] to request discharge from A-Pius and request to be placed with another home health agency that has a home health aide assist immediately with bathing needs and not interfere with patient care. Per patient and son, both requested to continue with A Plus Home Health, and continue to request SN [redacted] LVN as primary nurse. All of this was explained to [redacted] on June 16, 2017, patient's daughter, who was not present when patient requested to continue with A—Pius, and son agreed to bathe patient until another home health aide was available, [redacted] was explained that the patient had an option of discharging A-Pius Home Health and getting another agency, to which she stated she wouldspeak to her father and would call the agency back with his decision. [redacted] did not call back with any request for discharge. All information was given to [redacted], who is the MSW for patient at VA [redacted] , and per [redacted], patient has been identified as competent and continues to make his own decisions for his care and can decide he wants to continue with A-Pius services. [redacted] and [redacted] of APS, were notified that patient's home conditions were of concern to staff, but unless patient requests to be placed elsewhere, based his mental status, patient has his right to make his own decisions and where he wants to live and in what conditions.Please not: A-Plus Home Health continued to provide an aide to assist in bathing, resuming on 6/23/2017, of which only two visits were missed, per physician plan of care. A-Plus continued to treat patient for wound care to left great toe, which healed and discharged on 7/7/2017. Physical Therapy discharged patient on 7/5/2017 with all goals met and Occupational Therapy care for assistance in home on a long term basis. Patient and son were given 5 day discharge notice, as per Medicare guidelines, prior to discharge and both agreed that all goals had been met and signed notice in agreement.  [redacted]/CEO
