October 19, 2016Dear [redacted] ***:I am in receipt of your request for a response on the above referenced patient's second complaint dated September 30, [redacted] [redacted] states that she is rejecting the response given and is requesting a $refund for moneys due [redacted] has requested from dental office a response on the allegations of monies being withheld although on several occasions she has spoken with office manager and has received the same response, that the EOB's need to be mailed into the office and allotted monies paid to be posted into each patient's account Please be advised that [redacted] also put a complaint with her dental insurance carrier to dispute chargesOn October 3, dental office received a determination letter from her insurance stating the monies to be returned, were for $The refund given to [redacted] from A Plus Family Dental Care, PC, exceeded that amount [redacted] cashed the check on September 16, 2016.I ask that you discuss with [redacted] the determination letter sent by her insurance carrier and their findingsWe consider this complaint to be resolvedPlease advise, if this is not the case.Very truly yoursGeneral Counsel

of the Explanation of Benefits (EOB). At this time the office has not received payment for the services rendered on 5/27/2016 and 6/29/2016. Once copy of EOB is received it will state information on patients deductibles/copayments. We will have a response for you within the next two weeks.I trust that this will be satisfactory.If you have any questions, please contact me.Very Truly

I am rejecting this response because:  There still a balance of 12.00.  A P dental received a payment from my insurance company which left me  out of pocket 266.00I made a 480.00 payment. The check should amount should have  been 212.00 not 202.00

October 19, 2016Dear [redacted]:I am in receipt of your request for a response on the above referenced patient's second complaint dated September 30, 2016.[redacted]  [redacted] states that she is rejecting the response given and is requesting a $12.00 refund for moneys due.  [redacted] has requested from dental office a response on the allegations of monies being withheld although on several occasions she has spoken with office manager and has received the same response, that the EOB's need to be mailed into the office and allotted monies paid to be posted into each patient's account.  Please be advised that [redacted] also put a complaint with her dental insurance carrier to dispute charges. On October 3, 2016 dental office received a determination letter from her insurance stating the monies to be returned, were for $142.00. The refund given to [redacted] from A Plus Family Dental Care, PC, exceeded that amount.  [redacted] cashed the check on September 16, 2016.I ask that you discuss with [redacted] the determination letter sent by her insurance carrier and their findings. We consider this complaint to be resolved. Please advise, if this is not the case.Very truly yoursGeneral Counsel
