[redacted]I just wanted to say after last couple visits have no desire to return to winning smile . I went to another dentist to have some filed off as I was in pain after last visit to winning smile. Winning Smile nevr checked to see if it fit right with the carbon paper and it had a high spot.]

See attachment for original.It is hard to know where to start with this much incorrect information, but I will start at the beginning. The patient's tooth was first prepared for a crown due to a large filling going bad on the tooth. This was on November 19, 2012. A new filling was placed and the...
tooth was prepared for porcelain layered over gold crown. On November 28, 2012 the crown was cemented on. The “chipped" cap in between was just a temporary. On December 19, 2012 the patient reported with chipped porcelain and the crown was removed and prepared for a new crown. There was no charge for the new crown, The new crown was seated on January 9, 2013 and everything was OK at that time. As an aside, [redacted] is my assistant and does not "perform” any crown procedures. Going forward, the patient did not return for any maintenance checkups, cleaning, or x-rays. This is normally done at 6 month intervals to see if everything is OK and maintain the patient's mouth in an optimal condition.We did not see the patient again until May 8, 2015, over 2 years later, when the patient reported with a loose crown from eating caramel, something we always caution against. At this time we recemented the crown for $50, one-half our regular price of $100. The patient still would not come in for a checkup with screening x-rays to check for problems. On February 22, 2016 the patient came in with a lost crown and filling with tooth decay underneath. I informed the patient a new crown would be needed, but the patient refused. We did the only other option which was to place a new filling and do our best to shape the crown on it and cement it. The patient was informed that this was not the better treatment for this tooth. The patient has not returned since that time.The patient still has not paid a balance of $152 from the last treatment which we did a significant discount. I believe we have done our best for a patient who refused to come for regular checkups and did not follow directions. I always stand behind my work provided the patient does his part.
