Review: On 5/28/2014 I went to their facility for my first one hour physical therapy session. The physical therapist asked me to use the Pullie machine. It is a U-shape steel tube that runs 3 feet vertical on the wall. It has 3 i-hooks used for exercise stretch bands. As I was using this device, about half way through my 30-reps, the pipe came off the wall, hit my on the head, and knocked me to the ground. I sat on the ground for a few minutes and two of the employees came out of their offices when they heard the bang. One of them checked my head. The other employee went and picked up the pipe and brought it to me, to show me why it came off the wall. It was held in the wall by plastic anchors used for hanging pictures on the wall, and there was only one bolt at each end of the beam. I was so focused on my head hurting that I didn't even think about my neck and shoulders. They gave me a bag of ice for my head and had me continue my therapy. They put a head-pulse device on my neck, which aggravated my neck even more. That next day I called and cancelled my next appointment and I asked [redacted] for an Incident Report. I got a phone call from the owner [redacted] and he stated that he would write one and send it to me via mail. On 6/12/2014, I called them asking for the status of the Incident Report, and [redacted] and [redacted] told me that that was false, that I was not going to get one. [redacted] told me that it was because I said that I was "ok" right after the incident. But this was just a general statement and I hadn't had time to really think about what had just happened. Due to that device hitting my head, I felt worse. My neck was inflammed and swollen and I have been unable to relax. The pain is excruciating. A few days later, I noticed a bruise on my shoulder.Desired Settlement: I would like an acknowledgement of their negligence. I would like my Incident Report.
