This correspondence is in response to review/complaint #*** as submitted by Ms*** ***.I purchased *** *** *** in March of this year (2016). As part of the transition process it was required all licenses be transferred into my nameAs I began the process I was
informed the previous owner did not have the proper licenses and permitsSpecifically, the city of Glendale notified me they had denied my application for a Business License, as the home/building we were operating out of was not built to code and did not have the proper permits. Additionally, the City of Glendale denied my application for a Certificate of Occupancy, which is required by the AZ Department of HealthAs a result, I could not meet the licensing requirement of the Department of Health and the City of Glendale could assess a fine for every day I continued operating without a Business LicenseAfter exhausting all resources in an effort to correct the issues, a final determi*** on October 14, 2016, indicated I had no other option but to close my businessOn October 15, I verbally notified Ms*** of the situationAt this time, I also provided her the contact information of a Referral Agent that was aware of the situation and prepared to assist her in finding another placement for her mother in a timely manner. I also offered to assist Ms*** with packing and transporting her mother’s belongings to her new homeHowever, Ms*** declined my offers of assistance and threatened not to relocate her mother by October 31, It should be noted, other families involved used the resources of the Referral Agent, as recommended, and were successful in finding placement in a timely manner and without incidentI respectfully request the complaint be denied/removed, as our agency did nothing wrong and made every effort to assist and provide proper notification to Ms*** considering the circumstances as they wereThank you, Donna H***
