Revdex.com spoke with [redacted] from the company and she stated that the doctor has been on medical leave and they do not know when the doctor will be back. The doctor would need to approve and send the statement to the consumer.

Review: Since February 2013 I have made numerous telephone contacts with the medical receptionist for the Dr. requesting that I be provided a statement as to the current balance I owe the Dr. as I thought I had finally paid in full. I'm told each time I call that the Dr. handles the billing and that she will let the Dr. know. When I ask to speak with the Dr. "the Dr. is with a patient". My medical insurance has not been billed for my January and February 2013 visits, which is the last time I went to Dr. I hoped that this would be resolved either at the close of the 2013 tax year or by the latest when the taxes were filed.

I have sent letters to the Dr. requesting the same. with no response to date. Note the business moved to [redacted]. This is the last site that I received medical care.

The present mailing address where I have sent my yet to be answered letters is [redacted]. The business phone remains the same despite the multiple changes in the mailing addresses. It is [redacted].

In desperation I found a printout of my account as of 12/1/10. I then took all of my health insurance statements from 12/1/2010 thru 12/31/2012 and compiled a spreadsheet of my account. I did not include my 2013 visits since I have yet to have them be submitted to my health insurance. According to my records I am owed $1157.10 by Accident, Injury and Pain Wellness Center. Even though I last saw the Dr. was in February 2013, my credit card continued to have $100 a month withheld for a yet to be substantiated balance due. In August 2013 I disputed the $100 payment on my credit card.Desired Settlement: As stated in my letters of June 20 and July 1, 2014.

1. Submit my bills for 2013 to my health insurance company.

2. Once the bills are processed by my health insurance, send me a detailed statement of my account.

3. Bill me if I owe, or more correctly, pay me what I overpaid them.
