July 29, 2014 in Leesville, LA.On November 13, 2014, the claimant called and advised that she had completed treatment and will be forwarding her medical expenses. On November 21, 2014, the claimant forwarded her medical expenses relating to this claim. This was also referred to our Medicare vendor, [redacted] on November 21, 2014 to obtain the conditional payment letter from Medicare. Typically it has taken 90+ days to receive a conditional payment letter from Medicare. We are in contact with the claimant and have verbally explained the process to her on November 21, 2014, December 6, 2014, January 7, 2015, and February 09, 2015.
