Response:After reviewing the complaint, we looked into what happenedIt seems there was some miscommunication that took placeBelow is a list of events that took place [redacted] is the sales rep for the account and this is her email to her boss and meHi all, We’re in the clearUntil receiving [redacted] ’s email, I was unaware that the patient was disgruntledThanks to Outlook folders, I have all of the information you needHere is the timeline of events: April 1st, 2016: [redacted] called me and explained the patient’s unsuccessful attempts to mail in the rebateI called the rebate solutions numberSince the patient’s rebate had been returned to her, the rebate representative was unable to provide a resolutionIn order to provide a resolution, I offered to [redacted] that I send a SRF for a 3mL Latisse sample (not for resale) to [redacted] The 3mL was to be given to the patientIn the event that other patients run into the same problem, I sent a SRF for 3mL Latisse samplesMay 27th, 2016: The SRF for Latisse samples was signed and faxed to AllerganMay 31st, 2016: [redacted] ***, an Allergan representative, emails me to notify me that [redacted] (Dr [redacted] ***) faxed an SRF, which has expiredShe suggested that I create a new SRFI immediately create a new SRF for Dr [redacted] at [redacted] I email [redacted] to let her know that she will need to submit the new SRF to Allerganwithin daysJune 1st, 2016: Dr [redacted] with ***l [redacted] responds to my email via [redacted] ’s address, and notifies me that [redacted] no longer works at the practiceShe instructs me to email [redacted] ***, an employee of the practiceI forward the current SRF to [redacted] ***, and instruct her on how to submit the SRFJune 2nd, 2016: [redacted] submits the SRFJune 3rd, 2016: I receive a shipment confirmation via emailThe Latisse samples have been sent to [redacted] Please let me know if there is any further assistance or information that you would like for me to provideWould you like for me to contact Dr [redacted] or Dr [redacted] regarding this matter? [redacted]’s email, I was unaware that the patient was disgruntled. Thanks to Outlook folders, I have all of the information you need. Here is the timeline of events: April 1st, 2016: [redacted] called me and explained the patient’s unsuccessful attempts to mail in the rebate. I called the rebate solutions number. Since the patient’s rebate had been returned to her, the rebate representative was unable to provide a resolution. In order to provide a resolution, I offered to [redacted] that I send a SRF for a 3mL Latisse sample (not for resale) to [redacted]. The 3mL was to be given to the patient. In the event that other patients run into the same problem, I sent a SRF for 4 3mL Latisse samples. May 27th, 2016: The SRF for 4 Latisse samples was signed and faxed to Allergan. May 31st, 2016: [redacted], an Allergan representative, emails me to notify me that [redacted] (Dr. [redacted]) faxed an SRF, which has expired. She suggested that I create a new SRF. I immediately create a new SRF for Dr. [redacted] at [redacted]. I email [redacted] to let her know that she will need to submit the new SRF to Allerganwithin 30 days. June 1st, 2016: Dr. [redacted] with [redacted]l [redacted] responds to my email via [redacted]’s address, and notifies me that [redacted] no longer works at the practice. She instructs me to email [redacted], an employee of the practice. I forward the current SRF to [redacted], and instruct her on how to submit the SRF. June 2nd, 2016: [redacted] submits the SRF. June 3rd, 2016: I receive a shipment confirmation via email. The Latisse samples have been sent to [redacted]. Please let me know if there is any further assistance or information that you would like for me to provide. Would you like for me to contact Dr. [redacted] or Dr. [redacted] regarding this matter?
