Dear ***I just spoke with [redacted] to renew our membershipHe reminded me of our outstanding complaint with [redacted] and I asked him if I could explain what happened, then he said I should tell you.This has been ongoing and we have complied with everything that he has requested the moment he has requested it.However the ball is now in his court .In order to finish the issue, [redacted] needs to see his doctor for an evaluation so the MD can fill out the form for us to billHe is refusing to go to the doctor and now has stalled the process for us to complete the billing that he has requested of us.So long story short, when he visits the doctor and we receive the form we need to complete the process, we would be more than happy to bill and close this case.If you have questions call me.Thank you [redacted] ,Owner###-###-####
