In September of [redacted] contacted our office to schedule her daughter an appointment to see the doctorOn September 22, 2015, her daughter was seen in our office by DrCostanzaAt the end of the appointment, by the request of [redacted] ***, Dr Constanza spoke with her (***) and told her his findings and that he adjusted her upper and lower retainers and quoted her a price of a new upper and lower retainer should she decide to obtain themAt no time did [redacted] complain that DrCostanza adjusted her daughters retainers with out her permissionPer the original contract dated and signed by [redacted] on January 12, 2009, it states that her contract fee includes two years of retainer adjustments and visitsher daughter was debanded and upper and lower retainers were inserted on May 24, Since [redacted] left the office with out checking out at the front desk, we were unable to discuss and ask for the charge of the office visitA bill in the amount was sent to her for the office visit on September 22,it was explained to her by three different employees over the phone (including Dr Costanza himself that she was charged for an office visit since it had been beyond the years of debanding as stated in her contractIt is very unfortunate that the complaint would be filed when Dr Costanza saw her daughter as requested by [redacted] ***A service was rendered per her request, and Dr Costanza's time is certainly worth At this time we will remove the charge for the office visit on September 22, and any and all office visits going forward will incur a fee depending on the service that is rendered

At no time did [redacted] complain that Dr. Costanza adjusted her daughters retainers with out her permission. Per the original contract dated and signed by [redacted] on January 12, 2009, it states that her contract fee includes two years of retainer adjustments and visits. her daughter was debanded and upper and lower retainers were inserted on May 24, 2012. Since [redacted] left the office with out checking out at the front desk, we were unable to discuss and ask for the charge of the office visit. A bill in the amount 55.00 was sent to her for the office visit on September 22,2015. it was explained to her by three different employees over the phone (including Dr Costanza himself that she was charged for an office visit since it had been beyond the 2 years of debanding as stated in her contract. It is very unfortunate that the complaint would be filed when Dr Costanza saw her daughter as requested by [redacted]. A service was rendered per her request, and Dr Costanza's time is certainly worth 55.00. At this time we will remove the 55.00 charge for the office visit on September 22, 2015 and any and all office visits going forward will incur a fee depending on the service that is rendered.
