The complaint filed by Mrs [redacted] is based on lies and projecting her behavior onto our office staffWe have video and audio from her last visit to our officeWe also have at least three witnesses that can attest to the fact that she used abusive and obscene language towards our office managerOur office deals with drug addicts on a daily basis since we are a [redacted] Clinic, but we will not stand for rudeness or verbal abuseAs to her payment in January, she had a balance from DecemberIn fact, she still owes us $She also failed a urine drug screen, and missed appointmentsShe also missed appointments with her counselor, Mr[redacted] Due to her failed urine test, missed appointments, and not being able to locate her thru her phone or thru her counselor (who did not know where she was), she was dismissed from our clinicShe has a balance of $on her accountShe can send the payment thru mailWe do not want her in our officePlease remind her that we have cameras, video and audio.Thank you, [redacted] , MDcc: [redacted] , LADC
