Code of ethics violations for Clinical Director - Isaac V, LMFT, including: 1)Multiple relationships - treated all family members while excluding me from therapy without explanation - ultimately ending with divorce and family dissolution - it's an ethical violation to treat in this manner. 2)Abuse of therapeutic relationship - no completion of couples sessions as promised. 3)Client autonomy in decision making - suggestions to me about my spouse were that he wouldn't be able to support me - ultimately I believe he used the same suggestions to my husband and son who I no longer have relationships with. 4)Relationship beneficial to client - became friends with husband/son - allowing them to call/text for support & ultimately daughter who he asked to bring a friend and read his cards during their next session. 5)Non-abandonment - excluded me from couple/family therapy, never informed I wouldn't be involved. 6)Written authorization to release client information - after the divorce, texted all the diagnosis and reasons my husband and 21 year old son decided to "distance" themselves from me despite improved relations. Overheard multiple phone calls with spouse about my 21 year old son - know my son would not have signed a consent, even for his father. 7)Maintenance of records - multiple phantom bills with double bills - ultimately had to get the insurance company to retract payment. Inability to match charges with EOB's. Charging more than what was approved/discussed. 8)Scope of competence - claimed his experience was more than actually was - was not capable to handle the complexity of our marriage or family with only 1.5 years experience. 9)Financial integrity - made a house visit, stated it was ok to pay him directly - now I wonder if this would be against company policy. 10)Truthful representation of services - billing inappropriate codes for the treatment asked for and received. 11) Specialization - husband told kids about divorce 2 weeks before me - the reason we were in counseling was to stop all inappropriate discussions with kids, especially without me present.

Formal Response to False Allegations made by Nicole J
1. Multiple Relationships refers to relationship types outside of a treatment setting (ACA code of ethics, 3a, page 5). All family members were treated as clients only. At no point was another relationship entered in such as business, social, or personal. All other family members requested individual treatment besides Nicole J. She decided to see another therapist. Nicole J took this out of context.
2. Patient Nicole J was advised that after individual sessions, there may be a reassessment as to whether the couple desires to return into counseling. Due to the individual request of Nicole J’s husband, couples therapy was not provided.
3. Patient Nicole J had complete autonomy in decision making as evidenced by her open concerns of possible provisionary diagnoses. The decisions of her husband to divorce Nicole and her son to renounce their relationships are solely of their own volition and Patient Nicole does not desire to allow their autonomy.
4. Patient Nicole J took each individual rapport building exercise and boundary and took it out of context stating that the therapist were “friends”. Sincere and genuine rapport building is crucial to the client experience. All patients were advised to contact the therapist if they have any concerns freely and each family member besides Nicole has done so.
5. At the request of all Nicole Js family members. Nicole was not included in sessions and therapist honored each individual’s request to maintain safety and confidentiality away from Nicole.
6. At no time has the therapist texted full diagnoses to patient Nicole J. At the request of both father and son, who were working on their relationship, they have verbally agreed to share some information with each other.
7. Maintenance of Records. Patient Nicole J insisted to bill in a specific way to save money, which was not ethical. Patient Nicole was seeing therapist Brody B who is out of network for her insurance. Patient Nicole called her insurance and asked for a single case agreement for therapist Isaac V and acquired it. She specifically requested for her sessions with Brody B to be billed under Isaac V to avoid paying out of network fees. The billing department did not honor her request and she sent multiple emails asking for the billing to be “corrected”.
8. Patient Nicole J alleges that Therapist Isaac V only has 1.5 years experience. Patient Nicole clearly did not research that Isaac has been practicing in Florida as a clinician since 2013. She decided to look up the license information only in Utah in order to falsely allege that he does not have experience.
9. Financial Integrity: The house visit occurred and extra fees due to travel and time were assessed privately. Isaac is an independent contractor and executive director to Advanced Awareness Counseling and made appropriate decisions with extra fees payable to the therapist.
10. Truthful representation of services: The codes billed for were individual and family therapy. Patient Nicole did not agree with the billing code the professionals used who provided the service. A family session can be billed when the main work revolves around the family system.
11. Specialization: Patient Nicole is clearly upset about the decisions her children have made in keeping information from her. All these decisions were made by the husband and children themselves with full autonomy.
Summary:
As evidenced, patient Nicole does not desire to acknowledge what her husband and children have decided on their own. Due to lack of personal accountability, she desires to attach blame to the clinician. She has clearly misconstrued the ACA Code of Ethics in order to slander, attach blame, and hurt the livelihood of a clinician. This is the conclusion of the Formal Response to Nicole J’s Negative Review.
