HELLO I WAS A PATIENT OF DR MOHAMMAD N KHAN MD FOR A VERY SHORT TIMEI HAD WENT IN FOR CONSULTATION FOR A COLONOSCOPYAFTER THE VISIT I WAS SCHEDULE TO HAVE THE PROCEDUREBEFORE THE PROCEDURE THE PHARMACY WAS WAITING ON INSURANCE APPROVAL ON A PRESCRIPTION I WAS TOLDASK ME TO CONTACT MY PHARMACY INSURANCE TO SEE WHAT WAS NEEDED TO GET THE PRESCRIPTION APPROVED I CALLED MY PHARMACY INSURANCE AND WAS TOLD THAT THE PRESCRIPTION NEEDED A PRIOR AUTHORIZATIONASK ME IF I WILL CALL THE PHYSICIAN OFFICE AND HAVE THEM CALL US, FOR QUESTION BEFORE THE PRESCRIPTION CAN BE APPROVED I CALLED PHYSICIAN OFFICE ON 12/22/SPOKE TO A YOUNG LADY THAT ANSWER THE PHONE AND EXPLAIN TO HER THE SITUATION REGARDING THE PRESCRIPTION, WHICH WAS CALLED INTO THE PHARMACY HOW MY INSURANCE WANTED A CALL FROM THE PHYSICIAN OFFICE TO ANSWER SOME QUESTION THE YOUNG LADY WAS VERY RUDE AND WAS TOLD “FIRST YOUR INSURANCE WILL HAVE TO SEND US A FORM AND THE PERSON THAT FILL OUT THE FORM IS NOT IN THE OFFICE TODAY AND WILL NOT BE IN THE OFFICE TILL THE NEXT DAY” I REPLIED MY PHARMACY SAYS THEY JUST NEED SOMEONE FROM THE OFFICE OR THE PHYSICIAN TO CALL THEM AND ANSWER SOME QUESTION THE YOUNG LADY SAID IN A STRONGER RUDE TONE “AGAIN I TOLD YOU YOUR INSURANCE WILL HAVE TO SEND US A FORM” I REPLIED OKAY AND HUNG UP THE PHONE ON 12/27/I CALL THE OFFICE AGAIN SPOKE TO A YOUNG LADY AND I ASK TO SPEAK TO SOMEONE REGARDING WHO I CAN TALK TO ABOUT GETTING A PRESCRIPTION THAT WAS CALLED IN FROM DR KHAN AUTHORIZED BY HAVING SOMEONE CALL MY PHARMACY INSURANCE TO ANSWER SOME QUESTION THE YOUNG LADY PUT ME ON HOLDANOTHER LADY GETS ON THE PHONE IDENTIFY HERSELF AS BEING A NURSE I EXPLAIN TO THE NURSE THE SITIUATION REGARDING THE PRESCRIPTION THAT WAS CALLED INTO THE PHARMACY AND HOW MY PHARMACY INSURANCE WAS WAITING FOR A CALL FROM THE DR KHAN OFFICE TO ANSWER SOME QUESTION TO GET THE PRESCRIPTION APPROVED THE NURSE WENT ON TO TELL ME MY INSURANCE WILL HAVE TO SUBMIT A FORM TO THEM TO BE FILLED OUT I TOLD HER THEY ARE NOT GOING TO SEND A FORM, THE PHYSICIAN OFFICE WILL HAVE TO CALL THEM HAVING WORKED AT PHARMACY CALLED PCS IN SCOTTSDALE YEARS AGO AS A PHARMACY CLERK I KNEW THAT CERTAIN MEDICATION REQUIRED A PRIOR AUTHORIZTIONPHYSICIAN OFFICE WOULD CALL US THE PHARMACY INSURANCE FOR A PRIOR AUTHORIZTIONTHERE WERE QUESTION TO BE ASK REGARDING IF THE PATIENT TIRED AND FAILED OVER THE COUNTER MEDICATION OR OTHER MEDICATION ETC THE NURSE SHE PUTS ME ON HOLD FOR MINUTES AND THEN GET BACK ON THE PHONE AND TELL ME THAT SHE JUST SPOKE TO THE PHARMACY AND THAT MEDICATION IS NOT COVERED BY YOUR INSURANCE AND YOU MUST GET IT OVER THE COUNTER I BEGAN TO TELL HER THAT NOT TRUE BECAUSE I HAD SPOKEN TO MY PHARMACY INSURANCE AND IT IS COVERED IT JUST NEED A PRIOR AUTHORIZTION AND A CALL FROM THE PHYSICIAN OFFICEBEFORE I COULD EVEN FINISH TELLING HER WHAT WAS TOLD TO ME SHE WAS ALREADY TALKING OVER ME, AND YELLING THEN ARUGURING WITH ME AT THAT POINT I WAS SO UPSET AND SHOCKED OF THE LACK OF PROFESSIONALISM ON THE NURSE PART I TOLD HER TO CANCEL MY COLONOSCOPY PROCEDURE AND I WILL WRITE DRKHAN A LETTER EXPLAINGING WHY I CANCEL THE PROCEDURE AND WILL NO LONGER BE A PATIENT MINUTES LATER I GET A CALL FROM DR KHAN AND HE RESONDED TO ME “HELLO MY NURSE TOLD ME THAT YOU WOULD NOT LISTEN TO HER WHEN SHE WAS TELLING YOU THAT THE MEDICATION IS NOT COVEVERED AND YOU WILL HAVE TO BUY IT OVER THE COUNTER” I REPLIED DRKHAN YOUR STAFF HAS BEEN VERY RUDE TO ME AND THE MEDICATION IS COVERED IT JUST NEED YOU ARE SOMEONE FROM YOU STAFF TO CALL MY PHARMACY INSURANCE TO ANSWER SOME QUESTION SO THE MEDICATION CAN BE APPROVED ALSO IF THE MEDICATION WAS OVER THE COUNTER MEDICATION WHY WOULD YOUR STAFF CALL IN A PRESCRIPTION? no response I have called: phone number no longer functional I called chamber of commerce: They
are not a member and their only method of finding the company is the same as shown on computer searchThey
have received other inquiries due to non receipt of
their orderPhone (411) information does not show their business at the location indicatedAre they, in fact, in business? If not, it should be noted on
their site information so people will not continue to
place orders they have no hope of receiving

old style', 'new york', times, serif background-">-- this was done via *** ***. You may still want to investigate The Sheboygan Bratwurst Co however...to see if any other folks have been affected with a loss of money paid to that company for items not provided to customer.
I have already sent my complaint to the SC Atty General's office ....so they will be investigating also. Thank you for your assistance
