January 16,2017Complaint ID Number: [redacted] Response from Advanced ChiropracticBy Stacey P***- Office manager We began treating the pt on 12/05/thru 1/22/When the pt presented to the office on12/05/his vehicle insurance was called and verified to us that the pt had medical coverage.However, that coverage is NOT unlimited and anything above and beyond is the pts responsibility.Unfortunately, unbeknownst to us at the time the pt had seen a different medical provider elsewherethat exhausted his benefits when he sought care forth is particular injuryHis insurance was billed on1/08/which returned a denial on 1/20/that confirmed his benefits had been exhaustedAll ofwhich he was made aware of at his next visit on 1/22/2015(hence the reason why he never rescheduled)that the balance would be his responsibilityI spoke with his wife [redacted] shortly afteron the phone and was asked to bill their health insurance which we happily agreed to once we receivedhis health insurance informationWhen we finally did receive the information and billed the insurancewe received a denial on the claims due to "3'' party responsibility" and therefore exhausted all ouravenues to seek payment from anyone other than the account holderAt that time the full bill becamedue and payable by the pt.I will also note that we are VERY upfront with our payment policy as stated in our financial agreementthat WAS signed by the patient before he was ever seen in our office on 12/05/which states in part"If my PIP becomes exhausted, I understand that I am required to obtain the services of an attorneywithin days of the notice of PIP exhaustion OR BEGIN REGULAR PAYMENTS ON MY ACCOUNTIhave read and fully understand my responsibility concerning the payment of the services rendered."We continued to bill the pt on a monthly basisWhich after months of non-payment/no contact wesent a day collection warning letter with our intention to send the bill to collectionsThat somehowgarnered their attention and I was called immediately by this wife once againI agreed to hold off withcollections proceedings to allow her time to make payment arrangements and even offered a 25%discount if paid in fullI never received a call back thus here we areAll of this is well documented in hiscase file as well as in his chart notesWhich along with the financial agreement can be released withwritten approval by the PatientIf you need any further assistance please let me know Thank you, Stacey P***

All of this is well documented in hiscase file as well as in his chart notes. Which along with the financial agreement can be released withwritten approval by the Patient.
