Complainant was seen by DrSteve W*** on June 27,
He came into the office for treatment and required either a root canal or extraction
Complainant opted for Root CanalThe Drbegan the
Palliative treatment(the first step of the root canal) to alleviate some of the discomfort and pain
Complainant was advised this was a temporary treatment and would require him to come back into the office within the next couple of weeks
July 11, complainant scheduled appointment for the completion of the root canal
Complainant then called the same day (July 11, 2016) to cancel appointment
Based on the cost of the procedure, the complainant was advised the co-pay and a certain percentage was due by a certain day and any outstanding balance must be paid in full in order to complete treatment
Based on the unpaid balance the consumers 8/31/was cancelled
Consumer had (and has to date) an outstanding balance that required his immediate attention
As of 9/13/there is an unpaid balance that is over days due and must be paid in full
